摘要
目的分析抗环瓜氨酸肽抗体(抗CCP抗体)、抗突变型瓜氨酸波形蛋白抗体(抗MCV抗体)和类风湿因子(RF)联合检测结果与磁成振成像结果相关性,探讨3种抗体在类风湿骨侵蚀性关节炎亚型诊断中的临床意义。方法收集300例类风湿关节炎患者,300例健康体检者血清标本,检测其抗CCP抗体、抗MCV抗体和RF,并行磁共振成像检查,对结果进行相关性分析。结果抗CCP抗体、抗MCV抗体和RF联合检测的灵敏度为92.0%,行磁共振成像检查灵敏度为97.3%,二者检查结果相关性良好,Kappa系数等于0.78。结论抗CCP抗体、抗MCV抗体和RF都是诊断类风湿侵蚀性关节炎亚型的良好指标,3种指标联合检测与磁共振成像具有中度一致性,可以替代磁共振成像检查,减轻患者经济负担,广泛应用于RA筛查之中。
Objective To analyze the correlation between the detection results of anti - cyclic citrullinated peptide ( anti - CCP) antibod- y, anti - mutated citrullinated vimentin ( anti - MCV) antibody, and rheumatoid factor (RF) and the examination result of magnetic resonance im- aging ( MRI), and to explore the clinical significance of these antibodies for the diagnosis of rheumatoid arthritis. Methods Serum samples from 300 rheumatoid arthritis patients and 300 healthy subjects were collected. Anti - CCP antibody, anti - MCV antibody and RF were detected. The Analysis of correlation between the detection of these antibodies and MRI result was performed. Results The sensitivity of combined detection of anti - CCP antibody, anti - MCV antibody and RF was 92.0%, the sensitivity of MRI was 97.3%, there was good correlation between them and Kappa coefficient was 0.78. Conclusion Anti - CCP antibody, anti - MCV antibody and RF are good serological markers for the diagnosis of rheumatoid arthritis, and the results of combined detection of these antibodies are correlated with result of MRI examination. The combined exami- nation of these serum indicators can be widely used for rheumatoid arthritis screening instead of expensive MRI.
