2型糖尿病病人应用不同类型胰岛素注射用针头的注射时间和停留时间的研究 被引量:3

摘要 [目的]探讨不同剂量成人胰岛素注射2型糖尿病病人中应用32G×4mm胰岛素注射用针头的注射时间和停留时间,避免病人发生胰岛素注射剂量不足的现象。[方法]选择2011年12月—2012年6月在我科住院注射胰岛素的2型糖尿病病人148例,按单次注射最大胰岛素剂量分为常规剂量组(≤20 U)和大剂量组(>20U)。两组病人均使用32G×5mm和32G×4mm胰岛素注射用针头进行胰岛素注射各3周,第1周和第4周病人确定注射时间和胰岛素最佳停留时间,第2周~第3周和第5周~第6周按规定的时间进行胰岛素推注和停留。于第3周末和第6周末进行果糖胺的测定,比较使用不同胰岛素注射用针头的注射时间和停留时间。[结果]在使用32G×4 mm胰岛素注射用针头大剂量注射胰岛素时,胰岛素注射时间和停留时间要延长(P<0.05);无论使用哪种针头,大剂量注射胰岛素时胰岛素停留时间都会延长(P<0.05)。[结论]大剂量注射胰岛素尤其在使用32G×4mm胰岛素注射用针头时,胰岛素注射时间和停留时间要延长。 Objective:To probe into the injection time and residence time of 32G×4mm insulin injection needle in patients with type 2diabetes accepting different dose of adult insulin injection,and avoid the shortage of insulin dose in patients.Methods:A total of 148cases of type 2diabetes inpatients were selected in our department from December in 2011to June in 2012accepting insulin injections,and were divided into conventional dose group(≤20 U)and high-dose group(20U)according to single injection of insulin dose.The patients in both groups used 32G×5mm,and 32G×4mm insulin injection needle for insulin injection respectively for 3weeks,the patients determined the injection time and optimal residence time of insulin at 1st week and 4th week,insulin bolus at specified time were carried out at secondthird weeks and insulin residence at 5th-6th weeks based on stipulated time.To measure the fructosamine at the end of the third week and the sixth week and to compare the injection time and residence time among use of different insulin needle.Results:Insulin injection time and residence time were extended when using 32G×4 mm insulin injection needles for high-dose insulin injections(P0.05);regardless of the needles,insulin residence time would be extended on large doses of insulin injection(P0.05).Conclusion:The high-dose insulin injections,especially in the use of 32G×4mm insulin injection needles,insulin injection time and residence time will be prolonged.
