摘要 目的探讨3D高清腹腔镜的临床应用价值。方法分析18例3D高清腹腔镜手术的临床资料,其中应用3D高清腹腔镜行胆囊切除6例,胃癌、肛管癌根治及阑尾切除、肾囊肿去顶术各1例,子宫全切4例,子宫次全切除、附件囊肿剥除各1例,子宫、附件全切+盆腔淋巴结清扫2例。经脐入路手术6例,其余12入路同传统腹腔镜方式。结果 18例患者全部顺利完成手术,无中转开腹,手术时间及术中出血量与传统腹腔镜相当,无出血、胆漏、尿潴留等并发症发生。结论 3D高清腹腔镜能提供三维立体感知,可实现精确操作,使难度大的手术得以简化,安全、疗效好,无明显并发症发生,值得临床推广应用。 Objective To investigate the clinical value of 3D HD laparoscopy. Methods The clinical data of 18 cases of 3D HD laparoscopy were analyzed, including six cases of cholecystectomy, each case of radical resection of gastric cancer and anal canal cancer, appendectomy and renal cyst unroofing, four cases of total hysterectomy, each case of subtotal hysterectomy and adnexal cystectomy and 2 cases of total hysterectomy with adnexal cystectomy and pelvic lymphadenectomy. 6 surgeries were transumbilical. The method of the other surgeries is the same as conventional laparoscopic approach. Results All the surgeries were completed successfully with no conversion to laparotomy. The operation time and intraoperative bleeding volume is quite equal to the conventional laparoscopy, without bleeding, bile leakage, urinary retention and other complications. Conclusion 3D HD laparoscopy provides three-dimensional perception and precise operation, to simplify the difficult surgeries. It is worth promotion.
