摘要
目的研究Twist与E-cadherin蛋白在宫颈炎、宫颈上皮内瘤变及宫颈癌组织中的表达水平,分析其在宫颈癌中的作用。方法选取宫颈炎(20例)、宫颈上皮内瘤变(40例)和宫颈鳞状细胞癌(40例)的石蜡标本为研究对象,采用免疫组织化学方法检测Twist与E-cadherin蛋白在上述宫颈组织中的表达情况,分析其临床意义。结果①宫颈细胞胞质、胞核均可见Twist蛋白表达,以胞质表达更明显,其阳性率随恶性程度增加而逐渐增多;E-cadherin蛋白主要在细胞膜表达,其阳性率随恶性程度增加而降低。Twist和E-cadherin蛋白在宫颈炎、CIN及宫颈鳞癌组织中表达差异有统计学意义(P<0.05)。②在宫颈鳞癌组织中,Twist蛋白在有淋巴结转移的病例中高表达,E-cadherin蛋白仅在部分无淋巴结转移的病例中少量表达。Twist和E-cadherin蛋白阳性率在不同淋巴结转移差异有统计学意义(P<0.05),而在不同FIGO分期、不同肿瘤分化程度以及不同淋巴血管间隙浸润的组织中差异无统计学意义(P>0.05)。经Spearman相关性分析,Twist蛋白与E-cadherin蛋白存在负相关性(r=-0.561,P<0.05)。结论在宫颈癌变过程中Twist蛋白表达逐渐增加,E-cadherin蛋白表达逐渐降低,其介导的上皮间质化及两种蛋白的表达水平有助于判断肿瘤分化程度及转移潜能。
Objective To investigate the expression levels of Twist and E-cadherin protein in cervicitis, cervical intraepithelial neoplasia and cervical cancer, and to analyze their roles in cervical cancer. Methods Paraffin-embedded samples of cervicitis (20 cases), cervical intraepithelial neoplasia (40 cases) and cervical squamous cell carcinoma (40 cases) were the research objects. The expression of Twist and E-cadherin protein was detected with immunohistochemical method in the cervical tissues, and the clinical significance was analyzed. Results ① Cervical cell cytoplasm, nuclei were detected with the expression of Twist protein, especially in cytoplasm, and the positive rate increased with the aggravation of malignancy. E-cadherin protein was mainly expressed in the cell membrane, and the positive rate decreased with increasing malignancy. Twist and E-cadherin protein expression in cervicitis, CIN and cervical squamous cell carcinoma has significant difference (P〈0.05) in cervical squamous cell carcinoma, Twist protein in the high expression of lymph nodes metastasis, E-cadherin protein only in part without lymph node metastasis cases less expression. The positive rates of Twist and E-cadherin protein in different lymph node metastasis had significant differences (P〈0.05), while no statistically significant difference was found in different FIGO stage, tumor differentiation degree and different organizations of different lymph vascular space invasion (P〉0.05). Spearman correlation analysis showed a negative correlation between Twist protein and E-cadherin protein (r=-0.561, P〈0.05). Conclusion In the development of cervical carcinoma, Twist protein expression increased gradually, and the expression of E-cadherin protein decreased gradually. Epithelial mesenchymal mediated by the two proteins and their expression levels help determine the degree of tumor differentiation and metastasis.
