利声显心肌超声造影对顿抑心肌血流灌注的研究 被引量:1

摘要 目的 了解用利声显进行心肌超声造影时顿抑心肌的灌注情况。方法  9条健康杂种犬通过结扎左冠状动脉前降支 (LAD)制作顿抑心肌模型 ,经静脉注入利声显 ,用HPSonos 5 5 0 0型彩超仪观察正常状态、LAD结扎时以及松解后不同时间的心肌显像。结果 基础状态心肌显像良好 ;LAD结扎时局部心肌显像不良 ;LAD松解后 2 0~ 30min时 ,顿抑心肌部分显像 ;6 0min时 ,较均匀显像 ,7只犬于 90min、2只于 12 0min时显像恢复正常。结论 用二次谐频、能量多普勒显像、心电间断触发等技术 ,利声显可清楚显示不同情况、不同时间心肌血流灌注状态 ,帮助我们识别心肌的存活性和间接判断冠状动脉分支的走行、分布等 。 Objective To observe and stunning myocardial opacification during myocardial contrast echocardiography (MCE) by Levovist. Methods 9 healthy mongrel dogs were constructed into stunning myocardial models by ligating left anterior descending branch (LAD) of coronary artery. The Levovist was injected intravenously, and the stunning myocardial opacification were observed in various conditons. Results Before LAD occlusion, the myocardial opacification was good. The contrast defect in partial myocardium appeared after ligation but returned gradually to normal (partial opacification in 20~30 minutes, relatively hemo opacification in 60 minutes and normal opacification in 90 or 120 minutes) after release of ligature. Conclusion Myocardial opacification can be clearly shown with harmonice twice, power doppler imaging and intermitent trigger electrocardiographic technique. MCE can help the examiner to distinguish myocardial viability and to judge indirectly the running direction and distribution of coronary artery branches. It opens a new prospect in the study of the coronary artery disease in the future.(Shanghai Med J, 2000,23∶525 527)
