摘要
目的 :提高完全性肺静脉畸形引流 (TAPVC)的诊断和治疗水平。方法 :回顾性分析 9例患者的临床资料。结果 :术前确诊 3例 ,余均为术中确诊。心上型 8例 ,心内型 1例。全组无死亡。8例随访 1～ 1 0年 ,心功能 (NYHA) 0～ 级。身体发育与活动能力与正常同龄人无异。结论 :完全性肺静脉畸形引流术前易误诊为房间隔缺损。采用左右心房联合切口矫治本病 ,保证足够大的肺总静脉与左心房吻合口 ,可获得满意的远期效果。
? Objective:To improve the diagnosis and treatment of total anomalous pulmonary venous connection (TAPVC).Methods:The clinical data of 9 cases of TAPVC who were operated from 1984 to 1998 in our hospital were retrospectively analysed.Results:Three patients were diagnosed before operation;rest of them were diagnosed during operation.There was no death.Postoperative low cardiac output and left heart failure were present in 5 and 3 cases respectively.Eight cases were followed up 1 to 10 years.The cardiac function of them was 0~Ⅰgrade.Their physical development and performance were comparable to the normal men of same age.Conclusion:TAPVC was readily misdiagnosed as ASD before operation.Anastomosis between common venous trunk and left atrium throught combined left and right atrial incision is thought to be a best approach.The adequate size of anastomosis can get long good outcome. 〔
