基于影像表现预测骨关节炎进展 被引量：2

摘要 目前常用X线检查评估骨关节炎(OA)进展,MRI可了解OA初期至晚期关节功能丧失阶段进展规律。最新研究表明,MRI检测到的关节结构异常变化,如软骨缺损和骨髓水肿增加,软骨体积和厚度减少,软骨成分改变等可用于评估OA进展。肥胖、肌肉萎缩、关节内外翻、代谢性疾病、炎症和关节疼痛等临床危险因素及软骨损伤、骨髓水肿、半月板病变、滑膜炎等影像学特征等关节结构因素均与OA进展有关。改善这类危险因素,可减缓疾病进展;利用MRI监测疾病进展,发现新的危险因素,并对危险因素进行干预,有助于阻断OA进展。对处于高危险因素人群进行临床试验研究,可减少试验样本量和随访时间,增强临床试验研究效率。该文主要介绍OA进展的最新流行病学和临床研究证据,以及与OA进展相关的危险因素。
