摘要
股骨头坏死是一种常发生于中青年人群的髋关节高致残性疾病。常用的保留关节的手术包括髓芯减压坏死骨刮除术,骨形态发生蛋白、自体骨髓干细胞植入术,不带血管蒂的松质骨、皮质骨植骨术,带血管蒂的髂骨移植术,吻合血管的游离腓骨移植术,多孔钽金属骨小梁棒植入术以及股骨近端旋转截骨术等。目前,各种改良的髓芯减压术被证明是股骨头坏死保留关节治疗中最常用的手段。股骨头坏死保留关节手术主要适用于股骨头坏死早中期,选择恰当的保留关节手术可避免或推迟行人工关节置换术。
Osteonecrosis of the femoral head (ONFH) is a disabling clinical entity affecting young aduhs that usually leads to destruction of the hip joint. Surgical management alternatives for ONFH vary from joint salvaging procedures including proximal core decompression sequestrectomy, bone morphogenetic protein or autologous bone marrow stem cells transplantation, core decompression and replacement with non-vascularized cancellous or cortical bone grafting, vascular-pedicle iliae bone grafting, free vascularized fibular grafting, porous tantalum implant and femur rotational osteotomy. The most common procedures used to save the osteonecrotic femoral head are various improved techniques for performing core decompression. Joint salvaging procedures for ONFH are mainly suitable for early and medium stage ONFH. Selecting appropriate retention joint operation can avoid or delay total hip arthroplasty.
