七氟醚控制性降压对脑氧供需平衡的影响 被引量:2

摘要 目的 :观察七氟醚降压对脑氧供需平衡状态的影响 ,并与硝普钠 (SNP)进行对比。方法 :择期手术病人 36例 ,随机分为Ⅰ组七氟醚降压组、Ⅱ组七氟醚常压组和Ⅲ组SNP降压组 ,Ⅰ组增加七氟醚吸入浓度 ,Ⅲ组静滴SNP溶液 ,降低血压持续 40分钟 ,连续监测MAP、HR、及ECG ,并同步采集桡动脉和颈内静脉血行血气分析 ,计算脑氧摄取率 (ERO2 ) ,动 静脉氧含量差 (Da jvO2 )。结果 :Ⅰ组降压后ERO2 、Da jvO2 呈降低趋势 ;而Ⅲ组ERO2 、Da jvO2 显著增加。结论 :与SNP相比 ,七氟醚对脑有保护作用 。 Objective:To evaluate the efficacy and the effects of hypotension induced by sevoflurane or sodium nitroprusside (SNP) on balance of cerebral oxygen supply and consumption.Methods:Thirty six patients were randomly divided into three groups.Group Ⅰ,sevoflurane induced hypotension;group Ⅱ,sevoflurane maintained normal arterial tension;group Ⅲ,SNP induced hypotension.In group Ⅰ and Ⅲ MAP was decreased for 40 min either by sevoflurane inhalation or by nitroprusside infusion.MAP,HR,ECG were monitored continuously and radial arterial blood samples and jugular blood samples were taken simultaneonsly for blood gas analysis.Da jvO 2 and ERO 2 were calculated.Results:In group Ⅰ Da jvO 2 and ERO 2 were reduced during induced hypotension,but in group Ⅲ Da jvO 2,ERO 2 increased significantly.Conclusion:Compared with SNP,sevoflurane was safer for controlled hypotension in respect of cerebral metabolism.
