经皮内镜下胃造瘘术的临床应用价值 被引量:7

Analysis on the clinical value of percutaneous endoscopic gastrostomy

摘要 目的探讨经皮内镜下胃造瘘术(PEG)对长期全胃肠内营养患者的应用价值。方法 PEG组65例,鼻饲(NGF)组46例,比较2组患者置管前和置管后1个月、3个月及6个月的体质量指数(BMI)及血清白蛋白水平,同时比较置管前后患者吸入性肺炎、反流性食管炎的发生率;制定生存质量调查表,对比分析2组生存质量。结果 2组患者BMI及血清白蛋白水平在术后较术前均显著增高(P<0.05),PEG组与NGF组比较,术后3个月内BMI及血清白蛋白水平无显著差异,但与术后6个月时相比差异显著,PEG组均显著高于NGF组(P<0.05)。2组吸入性肺炎及反流性食管炎发生率在术后1个月内无显著差异,3个月后有显著差异。结论 PEG可以显著改善不能经口进食患者的营养状况,减少吸入性肺炎及反流性食管炎的发生,从而改善患者的生活质量,是长期胃肠内营养支持的有效途径。 Objective To explore the clinical value of percutaneous endoscopic gastrostomy(PEG) for patients with long term gastro-enteral nutrition.Methods The body mass index(BMI) and serum albumin of PEG group(65 cases) and NGF group(46 cases) were compared before the operation and after operation for 1 month,3 months and 6 months.The rates of aspiration pneumonia and reflux esophagitis were compared between the two groups at the same time.And the quality of life was compared too.Results The level of BMI and albumin were obviously increased after operation compared with pre-operation(P0.05).The level of BMI and albumin have no difference between PEG group and NGF group within 3 months after operation,but they were obviously higher than that in NGF group(P0.05) 6 months after operation.The rates of aspiration pneumonia and reflux esophagitis in the two groups were of no difference within 1 month but they were obviously different after 3 months.Conclusion PEG can improve the nutrition,reduce the occurrence of aspiration pneumonia and reflux esophagitis,thus improving the quality of life of patients with long term gastro-enteral nutrition.It is an effective method for long term gastro-enteral nutrition.
