摘要
目的探讨微型钛板联合侧向拉力螺钉技术治疗髁突囊内矢状骨折的方法与疗效。方法 13例患者(16侧)髁突囊内矢状骨折经耳屏前切口入路复位后,5侧用2组4孔微型钛板固定,8侧用1组4孔微型钛板加1颗拉力螺钉固定,3侧用2颗拉力螺钉固定,术后复查螺旋CT三维重建。结果平均追踪6个月,全部患者疗效满意,无开口偏斜、关节区疼痛、弹响。3例患者术后轻度张口受限,经开口训练后改善,2例患者术后咬合关系欠佳,经弹性颌间牵引后咬合关系恢复正常。结论针对不同类型的髁突囊内骨折,结合微型钛板及侧向拉力螺钉的优点采用不同的固定方法,对于治疗髁突囊内矢状骨折方便可靠、疗效明确。
Objective To evaluate the efficacy of lateral lag screw and mini plate in fixing intracapsular sagittal condylar fractures.Methods 13 cases（16 joints） were reset through the anterior tragus incision approach.Among the 16 joints,five joints were fixed by using two groups of mini plates;eight joints were fixed by using one group mini plate and one lateral lag screw;three joints were fixed by using two lateral lag screws.Curative effects were observed by spiral CT.Results The favorable results were obtained in all cases within six months.All patients were satisfied with the clinical results without any obvious function disturbances.Three cases were of slightly limitation of mouth opening and they were improved by opening exercise,and two cases were of poor malocclusions they were corrected by resilience intermaxillary traction.Conclusion According to the advantages of lateral lag screw and mini plate,the application of lateral lag screw and titanium miniplatecould be an effective treatment in fixing different types of intracapsular sagittal condylar fractures.
