摘要
目的应用经颅彩色多普勒超声结合屏气试验观察女性绝经前、后大脑中动脉血流动力学变化,评价女性绝经前、后脑血管反应能力的变化。方法选取45例绝经后女性和55例绝经前女性,并分别与相同年龄段的男性作比较,经颅彩色多普勒记录双侧大脑中动脉彩色血流及多普勒频谱情况;嘱受检者作屏气试验,记录屏气末的多普勒频谱及屏气时间,测量屏气末双侧大脑中动脉血流动力学等参数的变化,计算屏气指数(BHI)。结果绝经后女性组BHI[(0.87±0.4)s-1]低于绝经前女性组[(1.61±0.3)s-1,P<0.01],也低于年轻男性组[(1.46±0.5)s-1,P<0.01]和非年轻男性组[(1.18±0.6)s-1,P<0.05]。非年轻男性组BHI低于绝经前的女性(P<0.05)。结论绝经后女性BHI的降低受激素的影响,绝经后女性脑血管反应性降低。
Objective To explore the cerebrovascular reactivity in postmenopausal women and premenopausal women by transcranial color Doppler (TCCD) and breath-holding test. Methods Transcranial Doppler ultrasonograph was applied to study the hemodynamic changes of middle cerebral arteries in 45 postmenopausal women and 55 healthy premenopausal women. The same measurements were recorded in two groups of healthy male subjects with age matched with postmenopausal women (45 subjects) and premenopausal women (55 subjects). The Doppler spectrum and breath-holding time were recorded, the hemodynamic changes were measured, and breath-holding index (BHI) was calculated at the end of breath-holding. Results BHI was significantly lower in postmenopausal women [ (0.87±0.4)s1 ] than that in premenopausal women [ ( 1.61±0.3 )s-1, P〈0.01) and in young men [(1.46±0.5)s-1,P〈0.01] and old men [(1.18±0.6)s-1 ,P〈0.05]. In the latter group, BHI was significantly lower than that in premenopausal women (P〈0.05). Conclusion The decrease of BHI in postmenopausal women is influenced by hormone levels. The cerebrovascular reactivity is reduced in postmenopausal women.
