摘要
目的讨论MR及MRCP对复杂型胆道结石的诊断价值。方法收集术前行肝脏平扫MR及MRCP检查、术后证实为复杂型胆道结石患者37例,分析术前MR及MRCP,探讨其结石分布情况及结石信号特点。结果结石位于S 3段31例(32.3%),S 2段21例(21.9%),S 7段16例(16.7%),S 4段11例(11.4%),S 6段7例(7.3%),S 8段7例(7.3%),S 1段5例(5.2%),S 5段2例(2.1%)。37例患者中结石T2WI上均表现为低信号,MRCP均显示为扩张胆管内低信号充盈缺损。其中27例患者结石T1WI表现为高信号(72.9%),10例为低信号10(27.1%),均伴有病变叶段肝内胆管扩张,1例合并脓肿,2例胆管癌。结论 MR及MRCP对于复杂型肝内胆管结石具有很高的敏感性和特异性,可以进行多方位、多序列成像,清晰地显示胆道系统的情况和结石的成分。
Objective To investigate the diagnostic value of MR and MRCP on complicate biliary stones. Methods 37 patients were recruited in our study. They underwent MR scan and MRCP scan on a 1.5 T MR scanner. The distribution and signal characteristics were investigated. Results The stones located at Segment 3 31 （32. 3%）, Segment 2 21 （21.9%）, Segment 7 16（16.7%）, Segment 4 11（11.4%）, Segment 6 7（7.3%）, Segment 8 7（7.3%//00）, Segment 1 5 （5.2 % ）, and Segment 5 2 （2.1% ）. All the stones of the 37 subjects were low signal on T2 WI, and showed filling defect on MRCP, 27 among which showed high signal on T1WI（72.9%）, 10 subjects showed low signal （27.1%）. All the 37 subjects were accompanied by dilation of intrahepatic biliary duct. One of the 37 subjects was accompanied with hepatic abscess, and two with cholangiocarcinoma. Conclusion MR and MRCP show high sensitivity specificity on complicate intrahepatic biliary stones and could show the condition of biliary system and ingredient of stones using multidimension and multi-sequences.
