摘要
目的研究不同对比剂用量和注射流速情况下CT肺动脉成像(CTPA)的影像质量,探讨256层螺旋CT肺动脉成像中对比剂应用的优化方案。方法 60例临床上怀疑肺动脉栓塞的患者随机分为两组A组和B组,对比剂用量和注射流速分别为:A组35ml对比剂+30ml生理盐水,注射流速5ml/s;B组25ml对比剂+30ml生理盐水,注射流速4.5ml/s。比较分析两组的肺动脉节段、上腔静脉、升主动脉的CT值以及肺动脉节段的影像质量。结果两组各肺动脉节段CT值差异无明显统计学意义(P>0.05),上腔静脉和升主动脉CT值差异有统计学意义(P<0.05);两组肺动脉影像质量均可满足诊断要求。结论应用256层螺旋CT进行肺动脉成像时,较低的对比剂用量和相对较低的注射流速条件下,肺动脉显示良好,降低对比剂肾病的发生率,可以满足临床诊断的需要。
Objective To compare the image quality of computed tomography pulmonary angiography (CTPA) required with contrast medium (CM) of different volume and inject-rate using a 256-slice CT scanner and to investigate the optimized delivery protocol of contrast medium in computed tomography pulmonary angiography. Methods 60 patients suspected pulmonary embolism undergoing computed tomography pulmonary angiography were randomly divided into two groups, each containing 30 patients. The volume of contrast medium was 35 ml with the inject-rate 5 ml/s in group A, meanwhile that of group B was 25 ml with the inject-rate 4.5 ml/s. All followed by a saline flush of 30 mL The CT values of pulmonary arteries, superior vena cava (SVC) and ascending aorta (AA) were compared respectively. Image quality of pulmonary arteries was evaluated. Results There was no statistical significance in the CT values of pulmonary arteries ( P 〉0.05), and there was statistical significance in the CT values of superior vena cava (SVC) and ascending aorta (AA) in two groups ( P d0.05), two groups of pulmonary artery image quality could be diagnostic. Conclusion The use of proper injection-related factors in CTPA, such as a decreased CM volume and a relatively low flow rate can enable pulmonary artery contrast enhancement with a 256-slice CT scanner.
