新生儿缺氧缺血性脑病5l例动态脑电监测分析

摘要 目的:探讨动态脑电监测(AEEG)对新生儿缺氧缺血性脑病(HIE)的诊断及预后评估价值。方法:收集2012年1~9月新生儿重症监护室(NICU)HIE病例51例,均行AEEG监测,分析其临床特征并做近期随访研究。结果:HIE患儿EEG异常率为72.5%。轻度、中度、重度HIE患儿EEG异常率分别为40%、83.9%、100%,中度及重度HIE与轻度HIEL比较,EEG异常率有极显著性差异。EEG异常程度越重,患儿不良预后(死亡或遗留后遗症)的发生率越高。结论:AEEG能正确反映HIE患儿的脑功能状态,可作为HIE的分度及脑损伤的一个重要监测指标,对于HIE的诊断及预后评价具有重要意义。AEEG尤其适用于NICU中不宜搬动的危重患儿,可降低临床风险。 Objective: To discuss the assessed value of the diagnosis and prognosis of ambulatory electroencephalogram ( AEEG ) to neonatal hypoxic-intensive encephalopathy ( HIE ) . Methods: We collected 51 cases of HIE hospitalized in the neonatal intensive care unit ( NICU ) from January to September in 2012.AII cases were monitored by AEEG and we analyzed the clinical features and did a recent follow-up study. Results: The abnormal rate of electroencephalogram ( EEG ) in cases with HIE was 72.5%. In mild, moderate, severe caese with HIE, abnormal rate of EEG was 40%,83.9%,and 100% respectively. Moderate and severe HIE compared with mild HIE, the difference of abnormaliy rate of EEG was very significant. The severer EEG abnormality,the higher the incidence of poor prognosis ( death or sequelae ) .Conclusion: The AEEG can reflect brain function state of patient with HIE correctly, is an important monitoring index in HIE indexing and brain damage and has important significance in the diagnosis and prognosis of HIE. The AEEG especially apply to critical children should not be moved in the NICU and can reduces clinical risk.
