摘要
目的 评价低能量红激光局部照射 (LPRLI)对血管损伤后重塑模式的影响。方法 雄性新西兰白兔 10 0只 ,随机分为球囊拉伤组和球囊拉伤加激光照射组。球囊拉伤右髂动脉后 ,应用激光球囊照射拉伤局部 ,总的能量密度为 0 9J/cm2 。分别于术后 3天、1周、2周、4周和 8周分批处死动物 ,原位固定后截取病变血管进行病理组织学和免疫组织化学检测。最后 1批动物在取材前进行髂动脉造影。结果 拉伤后 3天单纯拉伤组管腔横截面积和外弹力板围绕面积明显低于激光照射组 ,管腔狭窄程度显著高于激光照射组 (P <0 .0 5 ) ;单纯拉伤组术后 2周血管腔横截面积和外弹力膜围绕面积与术后 3天相比有所增大 (P <0 0 5 ) ,血管狭窄程度减轻 (P <0 0 5 ) ,术后 2~ 8周血管腔横截面积和外弹力膜围绕面积均进行性减小 ,狭窄程度逐渐加重 (P <0 0 5 )。在 8周时已显著小于正常血管外弹力膜围绕面积 (P <0 0 5 )。激光组术后 2~ 8周血管狭窄程度无明显变化 (P >0 0 5 ) ,外弹力膜围绕面积进行性增大 ,到术后 8周已显著大于正常血管的外弹力板围绕面积 (P <0 0 5 )。术后 1周内膜加中膜面积两组均出现明显增厚 (P <0 0 5 ) ,从第 2周开始 ,激光组内膜加中膜面积显著小于单纯拉伤组 (P <0 0 5 )。髂动脉造影结果显示 ,?
Objective To investigate the effects of a certain density illumination of low power red laser on the remodeling after arterial injury. Method One hundred male New Zealand white rabbits were randomized into two groups: balloon injury group (control group) and balloon injury followed by laser illumination group (treatment group: total illumination density:0.9 J/cm 2). To study the time course of morphological changes, all the animals were killed on 3,7,14,28 or 56 days respectively after the procedure and with local fixation the iliac arteries were harvested for further pathomorphometrical analysis. At the end of 56 days, animals received angiography before killed. Results Under microscope, the scores of iliac arterial injury were similar between the two groups ( P>0.05). On the third day after the procedure, recoils were seen in both groups, but the stenosis rate of balloon group were significantly higher than those of laser group (P<0.05). The areas embayed by extral elastic membrane were altered synchronously with the lumen areas and it was smaller of balloon group than of laser group (P<0.05). At the end of 2 weeks, the areas under extral elastic membrane in balloon group increased obviously compared with those of the normal arteries and the stenosis rate reduced compared with those on third day (P<0.05). From 14 to 56 days, the areas of lumen and under extral elastic membrane were both reduced increasingly in balloon group and at the end of 56 days the later was smaller significantly than those of normal arteries. In laser group, the lumen areas showed no changes through the whole time but the areas embayed by extral elastic membrane increased gradually and at the end of 56 days they were significantly larger than those of normal arteries. At the end of 56 days, the stenosis rate was significant higher in balloon group than those in laser group (P<0 05). Iliac arterial angiography showed the minimal lumen dia meters and lumen stenosis rates in laser group were significantly improved compared with those in balloon group (P<0.01). Conclusion Low power laser illumination in a certain density followed arterial injury could significantly improve the mode of arterial remodeling and reduce the arterial stenosis degrees after balloon injury.
