冠心病血瘀证血浆代谢组学研究 被引量:23

Plasma Metabolomics Study on Blood Stasis Syndrome of Coronary Heart Disease

摘要 目的采用气相色谱(GC)-质谱(MS)联用技术,寻找冠心病(CHD)血瘀证患者内源性代谢分子标记物。方法收集CHD血瘀证患者48例、CHD非血瘀证患者52例及健康对照组40例,运用GC-MS联用技术检测各组受试者血浆代谢产物,对代谢物定性分析后,分别采用单变量t检验和多变量蒙特卡罗树法(MCTree)对各组代谢物含量进行比较。结果各组受试者血浆内源性代谢物种类无差别,总计46种。定量单变量t检验结果显示,CHD患者与健康对照组比较,有32种代谢物含量差异有统计学意义(P<0.05或P<0.01);CHD血瘀证组与CHD非血瘀证组比较,胆固醇含量升高,木糖醇含量降低,差异均有统计学意义(P<0.05)。MCTree结果显示,健康对照组与CHD患者血浆代谢物完全分离,对二者分离贡献最大是L-缬氨酸、甘氨酸、丙氨酸、L-丝氨酸,且含量较健康对照组明显降低;血瘀证与非血瘀证之间有明显分离趋势,44种物质对分类有显著影响。结论血浆GS-MS代谢图谱能有效的从健康人中区分出冠心病患者、从冠心病患者中区分出血瘀证患者,这些含量存在差异和对分类有贡献的物质可能是CHD血瘀证诊断和辨证的潜在代谢标记物和客观指标。 Objective To search the endogenous metabolites moleculer makers in plasma of coronary heart disease (CHD) patients with blood stasis syndrome by gas chromatography (GC)-mass spectrometry (MS) technique. Methods Forty-eight CHD patients with blood stasis syndrome, 52 CHD patients without blood stasis syndrome and 40 healthy persons were involved in the research. The plasma metabolites were detected by GC-MS technique. The metabolite contents were compared with t-test and Monte-Carlo tree method. Results There were 46 types of metabolites without difference among the subjects. There were significant differences between CHD patients and healthy persons in the contents of 32 types of metabolites (P〈 0.05 or P〈0.01). Comparing with CHD patients without blood stasis syndrome, the cholesterol content was significantly increased but the xylitol content was significantly decreased in CHD patients with blood stasis syndrome (P〈0.05). The plasma metabolites were completely separated in healthy persons and CHD patients. L-valine, glycine, alanine and L-serine were the greatest contribution to the separation and their contents in CHD patients were significantly decreased as compared with those in the healthy persons. There was a significant separation trend between blood stasis syndrome and non-blood stasis syndrome and 44 types of metabolites had a significant effect on the classification. Conclusion The plasma GS-MS metabolite profiles can effectively distinguish CHD patients from healthy persons and distinguish blood stasis syndrome in CHD patients. The substances of different content and contributing to classification may be the potential metabolic marker and objective indicators for diagnosing as CHD with blood stasis syndrome.
