自制肝肿瘤微波治疗仪的实验研究和临床试用 被引量:18

Experimental study and clinical application of self-made microwave treatmental apparatus of liver tumor

摘要 目的 采用自制肝肿瘤微波治疗仪在超声导向下对肝组织进行微波辐射的实验研究和微波治疗肝癌的临床试用。方法 选择离体新鲜猪肝、健康动物 (猪、犬 )及 2例肝癌患者 ,在超声导向下对肝脏及肿瘤植入微波天线 ,以不同功率、不同时间作用于肝组织和肝肿瘤 ,解剖实验动物 ,观察声像变化规律。结果 离体肝及活体动物肝 ,在微波功率 2 0~ 70W ,作用时间 60~ 3 60s ,逐次搭配作用形成凝固性坏死灶 ,大小范围为 8mm× 5mm× 4mm~ 3 6mm× 2 9mm× 2 4mm ,形态呈椭球形。微波作用区中心温度 10 8℃ ,边缘温度 ( 5 0± 5 )℃。2例肝癌患者微波治疗获得成功。结论 微波辐射选用条件以 60W、180s为好 ,所形成的凝固区范围与国内外报道相似 ,证明自行研制的肝肿瘤微波治疗仪是成功的。 2例肝癌患者的微波治疗取得满意效果。 Objective To evaluate the experimental study and clinical application of self made microwave treatmental apparatus in liver tumor treatment conducted by ultrasound.Methods Microwave was transplanted in the liver of fresh dissected liver tissue, healthy animals (pigs and dogs) and tumor lesion of two patients with Primary hepatocellular carcinoma (PHC) conducted by ultrasound,to observe sonography change influenced by different powers and times.Results The ellipsoid necrosis lesions (8 mm×5 mm×4 mm~36 mm×29 mm×24 mm) of liver (in vitro and in vivo) were found within 60~360 seconds after the microwave implanted with 20~70 W.The temperature in the center and periphery of lesions were 108℃ and (50±5)℃ respectively. Two patients with PHC were treated successfully by the self made microwave apparatus.Conclusions Experimental study demonstrates that microwave in condition of 60 W and 180 s was better and the range of coagulation tissue was similar to that reported abroad. It proved that self made microwave liver tumor treatmental apparatus were successful and 2 patients with PHC were treated with microwave and achieved satisfactory results.
