摘要
目的探讨全髋关节置换对伴膝关节骨性关节炎患者术后生活质量的影响。方法选择2010年1月-2011年10月期间收入的诊断为髋关节骨性关节炎、髋关节严重功能障碍患者共54例,对比患者手术前后双下肢长度,髋、膝关节活动度,术后生活质量及VAS评分。结果患者手术前后髋关节、膝关节及双下肢长度差对比中显示,全髋关节置换组患者手术前后膝关节活动度改变差异无统计学意义,髋关节活动度、双下肢长度差均有显著改善,差异有统计学意义(P<0.05)。患者术后生理功能、生理职能、躯体疼痛、总体健康、活力、社会功能、情感职能均较术前有显著提高,差异有统计学意义(P<0.05)。患者术后VAS评分较术前显著降低,差异亦有统计学意义(P<0.05)。结论人工全髋关节置换术可以显著改善伴随有膝关节骨性关节炎的患者髋关节活动度及双下肢不等长。在行髋关节置换术后半年,明显提高患者生活质量,并降低疼痛评分。
Objective To investigate the influence of total hip replacement surgery on the postoperative quality of life in patients both with hip and knee osteoarthritis.Methods A total of 54 patients with hip osteoarthritis and hip serious dysfunction from January 2010 to October 2011 in our hospital were enrolled in this study.The length of lower limbs,hip and knee joint activity,postoperative quality of life and VAS were compared before and after the operations.Results After the operations,knee joint activity was not obviously improved,but the hip joint activity and lower extremity length were improved significantly（P0.05）;The postoperative physiological functions,physical functions,bodily pain,general health,vitality,social function,emotional function were also significantly improved（P0.05）.The VAS score decreased significantly（P0.05）.Conclusion The total hip replacement can significantly improve the hip joint activity and lower extremity length inequality in patients both with hip and knee osteoarthritis.The quality of life and pain scores were improved 6 months after the surgery.
