摘要
双向转诊对于解决我国城乡居民看病难、看病贵问题具有十分重要的作用,新医改更是明确提出要逐步实现双向转诊。首先从双向转诊的涵义入手分析了双向转诊所存在的问题,包括社区卫生技术水平较低、人员素质不高、上级医院希望留住病人以获取更大的经济利益、转诊标准不确定和医保制度不健全等。然后通过分析国外的双向转诊模式,提出了针对解决双向转诊问题的合理化建议:建立社区首诊制;提高社区卫生服务水平;合理调整不同级别医疗机构之间的经济利益;建立健全转诊制度,规范转诊流程;医保制度与双向转诊相结合。
Two-way referral plays a very important role in solving the difficulty of expensiveness medical treatment for urban and rural residents, and it is also explicitly emphasized to implement the two-way referral gradually in health reform. Firstly we explored the definition of the two-way referral and then analyzed the problem of the two-way referral, including lower quality of health technology and staff in community, the hospitals at higher levels want to retain the patients to obtain greater economic benefits, the definition of referral standard is not clear, medical insurance system is not perfect, and so on. Then we analyzed the two-way referral model in western countries and then proposed suggestions to solve these problems： establishment of community first contact care; improving the level of community health services, rationally adjusting the economic benefits of different hospitals, establishing and perfecting of referral system and normalizing the procedures of referral, and the combination of medical insurance system and the two-way referral.
