摘要
目的 探讨在鼻窦内窥镜引导下 ,经鼻腔直达蝶窦鞍底进行垂体腺瘤切除术的效果。方法 对 4例患者应用鼻窦内窥镜经鼻腔入路手术 ,其中 3例蝶窦腔有多个分隔者经双侧鼻腔切除中鼻甲后端进入蝶窦 ,1例经宽大侧鼻腔切除中鼻甲后端进入蝶窦 ,开放蝶鞍底切除垂体肿瘤 ,采用明胶海绵和切下的中鼻甲后端去其粘膜层后封闭鞍底。结果 术后半年头痛均消失 ,3例视力及视野恢复良好 ,4例均无内分泌症状 ,无脑脊液鼻漏、出血、感染等合并症。结论 经鼻内窥镜蝶窦垂体腺瘤切除术是一种进路短捷、创伤小、病人痛苦小的手术方法。
Objective To investigate the effect of pituitary adenomatomy under the guide of nasoendoscope and directly reached to the sphenoid sinus saddle shape bone.Method under the guide of nasoendoscope it entered into the nose and performed the operation successfully.Result The headache was cured after the operation in half a year.3 of them the vision and the field of vision were recovered better.One of them showed normal vision and normal field of vision before and after the operation .There were no any other complications such as te symptom of endocrine system,leak of CSF and infection,etc.Conclusion Sphnoid sinus pituitary adenomatomy under the guide of nasoendoscope is a good method with the advantage of short rout ,little wound and slight suffering.
