阿托伐他汀抑制二尖瓣置换术后患者大内皮素及氮末端脑钠肽原上调 被引量:1

摘要 目的评价术前给予负荷量阿托伐他汀对二尖瓣置换术后患者大内皮素(big-ET)及氮末端脑钠肽原(NT-proBNP)的影响。方法 50例行二尖瓣置换术的患者,随机分为阿托伐他汀负荷组和安慰剂组,每组各25例。阿托伐他汀组术前12小时给予阿托伐他汀80 mg口服,安慰剂组对应时间点给予安慰剂口服。分别于术前1 d、术后1 d、术后3 d、术后7 d留取静脉血,检测血中big-ET及NT-proBNP。结果阿托伐他汀组和安慰剂组术前血浆big ET-1和NT-proBNP的浓度均无差别(P>0.05),术后1 d、3 d、7 d均小于安慰剂组(P<0.05)。结论术前给予负荷量阿托伐他汀可抑制二尖瓣置换术后big-ET及NT-proBNP上调。 Objective To investigate the influence of orally administrated atorvastatin and the levels of big endothelium - 1 ( big ET - 1 ) and N - terminal pro - brain natriuretic peptide ( NT - proBNP) after mitral valve replacement with cardiopulmonary bypass (CPB). Methods 50 patients planned to receive mitral valve replacement under CBP were enrolled in the present study. Patients were divided into atorvastatin group and control group randomly, 25 patients in each group. 80 mg atorvastatin was administrated in the atorvastatin group, and placebo was administrated in the control group 12 h before the operation. The levels of big ET - 1 and NT - proBNP were measured 1 d before the surgery, 1st d, 3rd d and 7th d after the surgery. Results The levels of big ET - 1 and NT - proBNP were not different between two groups before operation ( P 〉 0.05 ), hut much lower in the atorvastatin group than in the control group after surgery ( P 〈 0.05 ). Conclusion By administring load - dose volume atorvastatin pro - operation, the up - regu- lation of big ET - 1 and NT - proBNP could be inhibited.
