摘要
目的探讨心率震荡(HRT)检测在慢性心力衰竭(CHF)诊断与治疗中的临床应用价值。方法 62例CHF患者(观察组)接受24 h动态心电图(Holter)检查,计算其HRT、心率变异性指标和心功能指标,并与50例正常健康者进行对照。结果观察组震荡初始值(TO)明显高于对照组,震荡斜率值(TS)明显低于对照组(P<0.01);TO与左心室射血分数(LVEF)、左心室舒张末期内径(LVED)、正常窦性心律R-R间期的标准差(SDNN)呈负相关,TS与LVEF、LVED、SDNN呈正相关(P<0.05)。TO越高、TS越低,其再住院率及病死率就越高。结论 CHF患者的HRT现象明显减弱,HRT检测有利于进行CHF危险分层和评价治疗预后。
Objective To explore the clinical value of heart rate turbulence（HRT） detection in chronic heart failure（CHF） diagnosis and treatment.Methods 62 patients with CHF（study group） received the 24h ambulatory ECG（Holter） check,the HRT,heart rate variability index and cardiac function were calculated;and 50 cases of normal healthy persons were selected as controls.Results Turbulence onset（TO） of the study group was significantly higher than that of the control group and turbulence slope（TS） was significantly lower than the control group（P0.01）;TO and left ventricular ejection fraction（LVEF）,left ventricular end diastolic internal diameter（LVED）,normal sinus rhythm,RR interval standard deviation（SDNN） was negatively correlated,TS and LVEF,LVED,SDNN was positively correlated（P0.05）.The higher the TO and the lower the TS,the higher the readmission rate and mortality.Conclusion HRT phenomenon in CHF patients is significantly reduced,and the HRT detection is beneficial for CHF risk stratification and the prognosis evaluation.
