50名健康人胃电参数及其影响因素的研究 被引量:22

摘要 目的 测定健康人胃电图 (electrogastrography ,EGG)参数 ,研究皮肤阻抗、电极位置不同对EGG参数的影响。方法  5 0名正常体型 (体重指数BMI为 2 2 .5kg/m2 ± 2 .3kg/m2 )健康志愿者 ,5h胃排空X线检查正常。应用双电极DigtrapperEGG ,电极置于腹部X线定位胃体窦的体表投影位置。记录时间为餐前 0 .5h、餐后 1h ,试餐 46 0KJ。其中各有 10名健康人在皮肤阻抗大于和小于 10KΩ及X线和标准位置 (脐与剑突连线中点旁开 45°)分别放置电极进行餐前 0 .5h的EGG检查。结果 健康人餐后EGG主频 (DF)和主功率 (DP)升高 (P <0 .0 5 ) ,餐后餐前功率比 (PR)为 2 .7± 0 .2。主频不稳定系数 (DFIC)餐后较餐前降低 (P <0 .0 5 )。餐前正常节律百分比 (85 .2± 1.7) % ,餐后 (86 .0± 2 .1) %。皮肤阻抗大于 10KΩ胃电正常节律百分比为 (36 .1± 5 .5 ) % ,小于 10KΩ为 (89.6± 3 .4) % (P <0 .0 0 1)。X线与标准位置的正常节律百分比为 (81.8± 3.2 ) %和 (85 .5± 3 .9) % (P >0 .0 5 )。结论 EGG检查信噪比 (皮肤阻抗 )是主要的影响因素之一 ,皮肤准备在EGG检查中十分重要。 Objective To invevestigate the normal parameters of EGG and the influence of skin impedance, electrode position on EGG in healthy subjects. Methods Fifty normal figure healthy subjects entered the study. All subjects had normal 5 hour gastric emptying. Test meal contained 490 KJ (50g hum, a piece of Yin Dou noddle, 400 ml water). According to the X ray film, the location of antrum and corpus were defined. The EGG recordings were assessed for 30 minutes in the fasting and 1 hour in the postprandial period. The influence of the skin impedance and electrode position, were studied in ten subjects. Results The DF and DP were higher in the postprandial than in the preprandial ( P <0.05). DFIC was lower in the postprandial. Impedance<10 kΩ, the DF in normal range were lower than in>10 kΩ( P <0.001). The position with X film compared with the standard position were similar in DF in nomral range ( P >0.05). Conclusions The skin impedance is the most important influencing factor in EGG study and must be lower than 10 kΩ. In normal figure subjects the electrode can use standard position and does not need X film or ultrasonography.
