外源性磷酸肌酸配合含血停搏液应用于心脏瓣膜替换术的临床研究 被引量:1

摘要 目的 :观察外源性磷酸肌酸 (CP)配合含血停搏液对心脏瓣膜替换手术期间的心肌保护作用。方法 :6 0例病人分为对照组和实验组各 30例。观察两组病人心脏自动复跳率及除颤次数、恢复窦性心律及心律失常的发生率、开放循环后辅助转流时间、术后正性肌力药物的应用、48小时内血浆心肌酶的动态变化。结果 :两组病人心脏自动复跳率、恢复窦性心律比较 ,实验组高于对照组 ;除颤次数、心律失常发生率、辅助转流时间及正性肌力药物的应用实验组明显小于对照组 ;心肌酶变化趋势显示对照组释放值高于实验组 ,而下降缓于实验组。结论 Objective:This study was designed to evaluate the myocardial protective effect of exogenous creatine phosphate(CP) added into cardioplegic solution inclusive blood during heart valve replacement surgery.Method:60 patients were divided into a control group( n =30) and an experiment group( n =30).The spontaneous recovery rate of heart beats,frequency of eliminating ventricular fibrillation,recovery of sinus rhythm and arrhythmia,assistant cardiopulmonary bypass(CPB)after heart opening,positive inotropic drugs administred and changes of serum enzymes including CPK,CPK MB and LDH were determined within 48 hours after the surgery.Results:The spontaneous recovery rate of heart beats and recovery of sinus rhythm,frequency of eliminating ventricular fibrillation and arrhythmia,assistant CPB time and positive inotropic drug administrated in the experiment group were significantly less than the control group.In the control group,the peak values of leakage of serum enzymes appeared earlier and receded later than in the experiment group.Conclusion:The data indicates that exogenous creatine phosphate added to the cardioplegic solution inclusive blood has better protective effect on the ischemic myocardium during heart valve replacement surgery.
