摘要
目的:探讨趋钙激素对糖尿病(DM)合并高血压患者发病的影响。方法:对象为40例DM患者,其中非高血压(DM 1组)21例、合并高血压(DM 2组)19例及15例健康对照者。对3组进行血清钙、磷、甲状旁腺激素(PTH)、降钙素(CT)、葡萄糖耐量试验(OGTF)及胰岛素(INS)和C-肽(C-P)测定,并计算PTH/CT比值和葡萄糖/胰岛素指数。结果:3组检测结果比较显示DM 2组血清钙降低、PTH升高、CT降低、PTH/CT比值升高、OGTT 2h胰岛素升高、葡萄糖/胰岛素指数降低(P<0.01)。单因素相关分析表明,平均动脉压(MAP)与PTH、PTH/CT比值有相关性;多因素逐步回归分析表明PTH/CT比值在DM高血压发病中占有明显作用。结论:PTH、CT、PTH/CT、比值研究对DM高血压的发病机理有重要价值。
Objective: To evaluate the effect of calcitropic hormone in patients with diabetes mellitus complicated with hyperten-sion. Methods:Serum calcium and phosphorus,parathyroid hormone (PTH) ,calcitonin (CT) ,OGTT,insulin (INS) ,and C-peptide (C-P) were measured in 15 healthy persons (control group) and 40 diabetic patients which included 21 non-hyper-tensives (group 1) and 19 hypertensives (group 2). The ratio of PTH/CT and the index of glucose/insulin were calculated. Results:The levels of serum calcium,CT,and the index of glucose/insulin in group 2 significantly decreased compared with the other two groups (P < 0.01), and the levels of PTH, insulin at OGTT 2 hours, and the ratio of PTH/CT in group 2 sig-nificantly increased compared with the other two groups (P<0.01). There was a positive correlation between mean arterial pressure (MAP) and the level of serum PTH,and PTH/CT analyzed by single factor correlation. The ratio of PTH/CT is a main factor in the onset of hypertension in diabetic patients analyzed by multiple progressive regression. Conclusion: The study of the levels of serum PTH,CT,and the ratio of PTH/CT has an important value in pathogenesis of hypertension in diabetic patients.
