摘要
目的探讨天疱疮患者的医院感染特征,为预防和控制医院感染提供参考依据。方法采用回顾性与前瞻性相结合的调查方法,对医院2002年3月-2012年3月收治的125例天疱疮患者发生医院感染、感染部位和感染病原菌及相关因素等进行分析。结果天疱疮患者发生医院感染24例,感染率19.20%,感染部位以下呼吸道居多占41.67%,其次为泌尿道感染占20.83%,皮肤软组织感染占16.66%,上呼吸道感染占8.33%;检出病原菌20株,检出率为83.33%,其中,革兰阳性菌8株占40.0%,革兰阴性菌9株占45.0%,真菌3株占15.0%;通过对医院感染相关因素进行分析,其中年龄、住院时间、抗菌药物预防性应用和侵入性操作等是导致天疱疮患者发生医院感染的主要因素,差异有统计学意义(P<0.05)。结论天疱疮患者医院感染的发生率高,应当针对相关因素采取合理措施,以有效预防其医院感染的发生,促进患者早日康复。
OBJECTIVE To explore the characteristics of nosocomial infections in the pemphigus patients so as to provide basis for the prevention and control of nosocomial infections. METHODS By means of retrospective survey combined with the prospective investigation, the clinical data of 125 pemphigus patients who were treated in the hospital from Mar 2002 to Mar 2012, including the incidence of nosoeomial infections, the infection sites, and the distribution of pathogens, were analyzed. RESULTS The nosocomial infections occurred in 24 of all the pemphigus patients with the incidence rate of 19. 20%, among which there were 41. 76% with lower respiratory tract infections, 20.83% with urinary tract infections, 16.66% with skin soft tissue infections and 8.33% with upper respiratory tract infections. Totally 20 strains of pathogens were isolated with the isolation rate of 83. 33% , including 8 （40. 0%） strains of gram-positive bacteria, 9 （45. 0%） strains of gram-negatlve bacteria, and （15.0%） strains of fungi. The analysis of the related factors for the nosocomial infections concluded that the age, length of hospital stay, prophylactic use of antibiotics and invasive operation were the main factors leading to the nosocomial infections in the pemphigus patients, with statistical significance （P 〈 0. 05）. CONCLUSION The incidence rate of the nosocomial infections is so high in the pemphigus patients that it is necessary to take reasonable measures aiming at the related factors so as to prevent the nosoeomial infections and to promote the early recovery.
