血浆1,3-β-D-葡聚糖诊断侵袭性真菌感染的meta分析 被引量:5

摘要 目的:采用meta分析的方法评价血浆1,3-β-D-葡聚糖检测在侵袭性真菌感染诊断中的应用价值。方法:通过计算机检索Medline、The Cochrane central register of controlled trials、中国期刊全文数据库(CNKI)等,收集发表的关于血浆(1,3)-β-D-葡聚糖检测诊断侵袭性真菌感染的队列研究或病例-对照研究,应用STATA11.0统计软件对诊断的敏感性、特异性和ROC曲线下面积(AUC)进行合并。结果:18项研究,共4657例患者纳入meta分析。Meta分析结果显示血浆(1,3)-β-D-葡聚糖诊断侵袭性真菌感染敏感性为83%(95%CI:76%~90%),特异性为:80%(95%CI 77%~82%);诊断实验ROC曲线下面积分别为:0.88(95%CI:0.85~0.91)。结论:血浆(1,3)-β-D-葡聚糖检测诊断侵袭性真菌感染有较高的敏感性和特异性,可作为临床上快速诊断侵袭性真菌感染的有效方法。 Objective : To evaluate the clinical efficacy of plasma ( 1,3 ) - β - D - Glucan assay in the diagnosis of i nvasive fungal infection(IFI) by meta- analysis. Methods: By searching Medline EMBSE,The Cochrane cen- tral register of controlled trials, CNKI et al, all the cohort or case - control studies about ( 1,3 ) - β - D - glucan assay in the diagnosis of invasive fungal infection (IFI) were collected to calculate the pooled sensitivity,specificity and area under the received operative curve(AUC) by using statistic software STATAI !.0. Results: Eighteen tri'aJs with 4657 subjects were included in this meta- analysis. The pooled sensitivity, specificity and AUC were 83% (95% C1:76% -90% ) ,80% (95% CI 77% - 82% ) ,0.88 (95% CI:0.85 -0.91 ) respectively. Conclusion: Plasma (1,3) -β- D- glucan assay has good sensitivity and specificity in distinguish IFI from non -IFI, which can be used as quick diagnosis of IFI in clinic.
