血清胱抑素C与肌酐、尿素氮联合检测在肾功能损害诊断中的运用 被引量:17

摘要 目的:探讨血清胱抑素C(CyC)联合肌酐(Scr)、尿素氮(Bun)检测在肾功能损害诊断中的临床意义。方法:收集96例确诊为肾脏损害的住院患者及52例健康体检者,分为健康体检组、继发性肾病组(包括高血压肾病和糖尿病肾病患者)、尿毒症组,分析其血清CyC、Scr、Bun值,采用t检验进行比较分析。结果:CyC、Scr、Bun的异常率分别为62.16%、52.70%、54.73%,其中CyC的异常率高于Scr和Bun;尿毒症组中CyC、Scr、Bun渐进显著性(双侧)分别为0.806、0.952、0.866,继发性肾病组为0.721、0.746、0.687,健康体检组为0.762、0.852、0.726。结论:胱抑素C是一个对肾功能损害敏感且诊断价值较大的指标,但其特异性较低,可与特异性较高的肌酐、尿素氮联合检测以提高检测肾功能损害程度的灵敏度和特异性。 Objective :To discuss the value of joint detection of serum Cystatinc C, creatinine and urea nitrogen for renal disfunction indexes. Methods: Ninety - six hospital inpatients with renal disfunetion and 52 healthy sub- jects were collected and divided into healthy control group, secondary nephropathy group and uremia group, and the levels of serum CyC, Scr, Bun in these groups were analyzed using t test. Results: The abnormal rate of CyC and Scr,Bun were 62.16% , 52.70% ,54.73%. The abnormal rate of CyC was higher than that of Scr and Bun; The progressive signifieances(bilateral) of CyC, Scr and Bun in uremia group were 0. 806, 0. 952 and 0. 866 respective- ly, in secondary nephropathy group were 0. 721, 0. 746 and 0. 687 respectively, in healthy control group were 0. 762, 0. 852 and 0. 726 respectively. Conclusion : CyC is a sensitive index for renal dysfunction with higher diag- nostic value, but because of the low specificity, it has to combine with the creatinine and urea nitrogen to improve the sensitivity and specificity of renal dysfunction detection.
