摘要
目的:讨论全髋关节置换术治疗老年股骨颈骨折与半髋关节置换术的临床疗效对比。方法:56例股骨颈骨折患者随机分为两组,全髋关节置换术组28例和半髋关节置换术组28例,分别对两组患者的手术时间、手术中出血量、手术后失血量、总输血量、住院时间、并发症情况、手术后肢体功能恢复情况及术后2年疼痛情况等进行比较。结果:全髋关节置换术组手术总时间明显长于半髋关节置换术组(P<0.05);两组患者的手术中出血量、手术后失血量、总输血量、住院时间、并发症情况、术后肢体功能恢复情况比较均无明显统计学差异(P>0.05),而半髋关节置换的患者疼痛程度明显高于全髋置换的患者(P<0.05)。结论:半髋关节置换术和全髋关节置换术治疗老年股骨颈骨折,短期疗效无明显区别,但是患者术后的生活质量,全髋关节置换术疗效优于半髋关节置换术。
Objective： To explore the clinical effect of total hip arthroplasty and partial hip arthroplasty for femoral neck fracture in elders. Method ：56 patients were randomly divided into the total hip arthroplasty group and the partial hip arthroplasty group. The time of surgery, amount of bleeding, blood transfusion, hospital stay, complications, recovery of affected limb, the pain after surgery were compared. Result： The time of surgery in total hip arthroplasty group was longer than the partial hip arthroplasty group { P〈0.05 }. There was no significant difference of the amount of bleeding, blood transfusion, hospital stay, complications and recovery of affected limb between two groups （ P〉0.05 }. The pain in the partial hip arthroplasty group was more serious than the total hip arthroplasty group { P〈0.05 ）. Conclusion： There is no significant differ- ence in short-term clinical effect of total hip arthroplasty group and the partial hip arthroplasty group for fem- oral neck fracture in elders. But, the quality of patients after surgery in the total hip arthroplasty group is bet- ter than the partial hip arthroplasty group.
