伤椎植骨置钉预防椎体矫正度丢失的临床研究 被引量:7

摘要 目的:探讨经伤椎植骨置钉对预防椎体矫正度丢失的效果。方法:回顾性分析2008年5月-2010年12月本院采用椎弓根内固定系统治疗的43例胸腰椎骨折患者的临床资料,经伤椎椎弓根椎体内植骨置钉20例(伤椎处置组),单纯后外侧植骨23例(伤椎未处置组)。对两组患者在不同时间段椎体压缩率及cobb角进行比较。结果:两组患者术前椎体压缩率及cobb角比较差异无统计学意义(P>0.05),术后即刻和术后15个月时伤椎处置组椎体压缩率及cobb角均小于伤椎未处置组,差异具有统计学意义(P<0.05)。结论:经伤椎椎弓根行椎体内植骨置钉结合椎弓根内固定治疗胸腰椎骨折可获得满意复位,能重建椎体高度且能长期维持,临床效果理想。 Objective: To assess the efficacy of the method in treatment of thoracolumbar fractures with bone graft and vertebral pedicle screw in fractured vertebra.Method: The clinical data of 43 cases with thoracolumbar fractures in treatment of pedicle screw internal fixation systems in author's hospital from May 2008 to December 2010 were analyzed retrospectively.20 patients in ( vertebral disposal group ) were treatment with bone graft and vertebral pedicle screw in fractured vertebra, 23 patients in (vertebral untreated group )were treatment with posterolateral fusion without implants. Two groups of patients in different periods of vertebral compression rate and cobb angles were compared.Result: There was no statistical significance compared with preoperative vertebral compression rate and cobb angles between two groups ( P〉0.05 ) .Immediately after operation and 15 months after operation, the vertebral compression rate and cobb angles of vertebral disposal group was lower than vertebral untreated group ( P〈0.05 ) .Conclusion: In the treatment of thoracolumbar fractures with bone graft and vertebral pedicle screw in fractured vertebra has a ideal clinical effect, it can achieved a satisfactory reduction, reconstructed vertebral body height and maintained for a long time.
