摘要
目的探讨超声引导下定位胚胎移植对体外受精-胚胎移植(in vitro fertilization and embryo transfer,IVF-ET)妊娠结局的影响。方法回顾性分析本生殖中心接收的首次IVF-ET新鲜胚胎移植3119个移植周期资料,以胚胎移植管顶端距子宫底的距离分为A、B、C、D、E 5组,比较各组间临床妊娠率、种植率、异位妊娠率。结果 A、B、C、D、E 5组的临床妊娠率差异无统计学意义(P>0.05);种植率分别为36.74%、35.32%、33.03%、30.45%和38.55%,A、B、E组之间,C组和D组之间差异均无统计学意义(P>0.05),A、B、E组种植率和异位妊娠率显著高于C组和D组,差异均有统计学意义(P<0.05)。结论临床妊娠率、异位妊娠率与移植管距子宫底的距离没有关系,种植率可能与胚胎质量有关系。
Objective To investigate the relationship between the distance of ultrasound-guided embryo transfer from the fun- dus（TDF） and the clinical pregnancy rate（PR） in the process of in vitro fertilization and embryo transfer（IVF-ET）. Methods Divid- ing 3119 cycles of IVF-ET into 5 groups aceorciing to the TDF：Group A with TDF 〈10 ram; Group B with TDF 10. 1 mm to 15 ram; Group C with TDF 15.1 -20 mm;Group D with TDF 20. 1 -25 mm; Group E with TDF 〉25.1 mm. Patients were divided into the clinical pregnancy rate, early abortion rate, late miscarriage rate, eetopie pregnancy rate and live birth rate were compared and analyzed among those five groups. Results Significant differences （ P 〈 0. 01 ） in the PR were observed among those 5 groups,which the clinical PR（ 34.47% ）of Group A was significantly lower than the other groups and live birth rate （26. 21% ）of Group A was significantly lower than the Group E. Conclusion The the distance of ultrasound-guided embryo transfer from the fundus influents the embryo implanta- tion, clinical PR and live birth rate. Decreasing the TDF resulted in significantly decrease clinical pregnancy rate as well as live birth rates
