摘要
目的:探讨镍钛记忆合金聚髌器治疗髌骨粉碎性骨折的疗效。方法:采用镍钛记忆合金聚髌器治疗新鲜髌骨粉碎性骨折25例,左侧15例,右侧10例;按照受伤机制分类:车祸伤9例,摔伤6例,高处坠落伤6例,打击伤4例;按照骨折是否伴有移位分型:粉碎不伴移位骨折14例,其中碎骨块≤5块者9例,>5块者5例;粉碎伴移位骨折11例,其中碎骨块≤5块者7例,>5块者4例。结果:25例患者随访12～30个月,平均18个月。骨折愈合时间6～12周,平均8周。术后患侧深静脉血栓形成1例,经溶栓治疗痊愈;术后无内固定松动、断裂及再骨折发生;随访无创伤性关节炎、关节僵硬、膝关节炎等发生。按疗效评定标准,优14例,良9例,可2例,优良率92%。结论:对于髌骨粉碎性骨折,采用镍钛记忆合金聚髌器固定可靠,疗效满意。
Objective：To investigate the of the treatment of comminuted patellar fractures with NiTi-.Patellar concentrator. Methods： Twenty five cases of fresh comminuted patellar fracture were treated with internal NiTi-Patellar concentrator fixation. The demography of the 25 cases are as follows. The mechanism of injury included car accident（9 cases）, tumble （6 cases）, fall （6 cases）, and direct strike （4 cases）. According to whether with displacement of fracture, there were 14 cases of comminuted fractures without displacement and 11 cases with displacement. In the former group, 9 cases were less than 5 fragments, 5 cases were more than 5 fragments; and in the latter group, 7 cases were less than 5 fragments ,4 cases were more than 5 fragments. Results： Twenty five cases were followed up with a mean period of 18 months （from 12 to 30 months）. All patients obtained complete bone union in 6 ~ 12 weeks. One case of affected limb deep vein thrombosis was cured through thrombolysis treatment. No case of implant loosening or refracture was found. According to the scoring system, 14 excellent, 9 good and 2 fair was obtained. The excellent and good rate was 92%. Conclusion： The NiTi-Patellar concentrator provided an ideal treatment for comminuted patellar fracture.
