The Changes and Significance of Serum 25-OH Vitamin D Level in Type 2 Diabetes Patients

摘要 目的:探讨上海地区初诊的2型糖尿病(T2DM)患者血浆25-OH维生素D(25-OHVitD)水平,以及25-OHVitD与β细胞功能、胰岛素抵抗的关系。方法:随机入组T2DM患者90例,年龄匹配的正常对照86例,留取血清及血浆标本测定血糖、胰岛素及血脂。使用液相色谱-质谱法测定血清25-OHVitD水平。在T2DM组中使用HOMA-IR评价胰岛素抵抗,HOMA-β反映早期相胰岛素分泌功能。结果:T2DM组血清25-OHVitD水平较正常对照组显著下降(P=0.03),25-OHVitD水平与HOMA-IR不相关(r=-0.08,P=0.59),25-OHVitD水平与HOMA-β正相关(r=0.4,P=0.03)。结论:T2DM患者存在25-OHVitD缺乏,且25-OHVitD水平影响β细胞功能。 Objective:To examine the 25-hydroxy vitamin D(25-OH VitD) concentration of patients with type 2 diabetes mellitus(T2DM),and the association of 25-OH vitamin D concentration with beta cell function and insulin resistance.Methods:90 patients of T2DM and control group with age matched and 86 normal glucose tolerance were randomly recruited.Their height,weight were measured and recorded.Their fasting serum and plasma sample were also collected for the measurement of serum lipids,plasma glucose,serum insulin.Insulin homeostasis model assessment of insulin resistance (HOMA-IR) and function of βcell (HOMA-β) were also assessed.25-OH VitD were measured in liquid chromatography/mass-spectrometric method and the concentrations in both groups were compared.Results:The level of 25-OH VitD in T2DM group were significantly lower than those in control group(P=0.03).The level of 25-OH VitD were correlated with HOMA-β(r=0.4,P=0.03),but not with HOMA-IR(r=-0.08,P=0.59).Conclusion:Patients of T2DM are lack of 25-OH vitamin D and the deficiency may influence islet β cell function.
