喉癌中Oct-4基因的表达及其临床意义

摘要 目的检测Oct 4基因在喉鳞癌组织中的表达,探讨其相关的临床意义。方法收集我院2007年1月~2010年12月耳鼻咽喉科手术切除标本48例,所有病例经病理证实为喉鳞状细胞癌,每例另取癌旁非癌喉粘膜组织作为对照。病例按照病理分化程度、有无淋巴结转移、临床TNM分期分组对照Oct 4基因的表达。RT-PCR检测Oct 4mRNA的表达,荧光定量PCR检测Oct 4基因的表达量。结果 Oct-4基因在喉鳞癌组织中的表达率为64.6%,荧光定量PCR结果也显示喉鳞癌标本中Oct 4基因的表达量为0.533±0.142,而周围正常对照组织未检测出Oct 4基因的表达(P<0.01)。Oct4基因在喉鳞癌高分化组阳性率为33.3%显著低于中、低分化组的69.0%(P<0.01);有淋巴结转移者阳性率为47.8%显著低于无淋巴结转移的80.0%(P<0.01);处于临床Ⅰ、Ⅱ期的患者Oct 4基因表达阳性率为47.4%,而Ⅲ、Ⅳ期的患者阳性率为75.9%,两者之间统计差异有统计学意义(P<0.01)。荧光定量PCR结果显示Oct 4基因的表达水平在喉鳞癌组织中会随着肿瘤分化减低、淋巴结转移和临床分期的进展而逐渐的增加(P<0.05)。结论 Oct 4基因在喉鳞癌的发生发展中起到一定的作用,可以用于肿瘤的分级以及判断预后等方面,具有显著的临床意义。 Objective To investigate the mRNA expression and clinical significance of OCT-4 in human laryngeal squamous cell carcinoma. Methods 48 surgical resection specimens from otorhinolaryngology between January 2007 to December 2010 in our hospital were enrolled in our study. All cases were pathologically confirmed squamous cell carcinoma and the paraneoplastic tissue as control. Cases in accordance with the degree of pathological differentiation, lymph node metastasis, TNM stage grouping control gene expression of Oct- 4.The laryngeal carcinoma tissues and adjacent normal laryngeal tissues were collected for RT-PCR and Real-time PCR analysis. Results The positive expression rate of Oct-4 was 64.6%. However, the Oct-4 can not be detected in adjacent normal laryngeal tissues by RT-PCR or Real-time PCR analysis. The mRNA expression of Oct-4 was significant related with histological grade, lymphatic metastasis, and clinical pathologic stage in human laryngeal carcinoma(P〈0.01). Conclusions The expression of Oct-4 gene had a closed relationship with the development of laryngeal squamous cell carcinoma.The high-level expression of Oct-4 mRNA might be used as the marks for estimating the degree of differentiation of laryngeal carcinoma.
