摘要
目的 观察承受不同程度热缺血的离体大鼠肝脏三磷酸腺苷 (ATP)的再生能力及相应的病理变化 ,从代谢和形态结构两个方面判断其活力。方法 SD大鼠 18只 ,按热缺血时间随机等分成 3组 :A组 ,0min ;B组 ,30min ;C组 ,6 0min。测定加氧低温灌流的离体肝脏31磷 核磁共振频谱 ,同时观察灌流前后肝组织病理变化。结果 灌流 30min时 ,A、B、C 3组大鼠肝脏ATP/亚甲基二磷酸(MDP)平均值为 0 .42 3、0 .387、0 .138(C组有 3例测不到ATP峰值 ) ,A、B两组差异无显著性 (P >0 .0 5 ) ;C组与A、B两组比较差异有非常显著性 (P <0 .0 1)。病理改变 :A组肝细胞轻度浊肿 ;B组还有散在肝细胞空泡变 ;C组 ,特别是测不到ATP的 3例肝脏有大片肝细胞空泡变。肝窦内皮细胞A、B两组存在轻度水肿 ,C组肿胀加重。结论 测定加氧低温灌流离体大鼠肝脏能量代谢能有效鉴别其活力。
Objective\ Study the ability of regenerating ATP of the rat livers of warm ischemia and the pathological changes during oxygenated hypothermic reperfusion and assess the viability of the rat livers by evaluating both metabolic and morphological alteration.Methods\ 18 SD rats were randomly and equally divided into three groups. In groups A, B and C, 6 rat livers were subjected to 0 , 30 , 60 min warm ischemia respectively. All the livers were perfused through portal vein with oxygenated cold modified Krebs Hensleit (mK H) solution for 30 min in a 31 P MRS machine where the 31 P NMR spectroscopies were carried out and were biopsied around perfusion for pathological examination. Results\ After 30 min perfusion, the mean ratio of ATP to methylenediphophate ( MDP) in the groups of A, B and C was 0.423, 0.387 and 0.138 respectively. The ability of ATP regeneration of the rat livers in group B was simillar to that in group A ( P >0.05) .However, in group C, 3 of 6 rat livers could not resuscitate this ability, and the ability of ATP regeneration in group C was significantly less than in the group A and group B (P< 0.01 ). The primary pathological change was edema of hepatocytes in group A. Besides edema of hepatocytes, slight bleb formation of a few hepatocytes in group B and severe bleb formation of a large number of hepatocytes in group C were found, especially in the 3 rat livers undetecting ATP. There was slight swelling of the sinusoid endothelial cells in group A and group B.There was more severe swelling in group C than in the other groups. The edema of both hepatocytes and endothelial cells reduced at different degrees after reperfusion.Conclusion\ Measurement of the ability of regenerating ATP of the rat livers of warm ischemia can effectively assess the extent of injury and the viability during oxygenated hypothermic reperfusion.\;
