摘要
目的:评价根据JCI标准制订的《社区卫生诊疗流程》在社区门诊诊疗过程中对社区全科医生医疗行为和患者满意度的实施效果。方法:对医务人员进行"以患者为中心"的JCI核心理念及《社区卫生门诊诊疗流程》规范培训,采用自制调查问卷对门诊医生的诊疗行为和患者满意度进行培训前后的调查分析。结果:经过培训,医生在询问病史、体格检查、病情告知以及患者对医生的满意度等方面均有明显提高,与培训前比较,差异有统计学意义(P<0.01)。患者就诊后是否按医嘱服药的依从性及规律性在培训前后无明显改变,差异无统计学意义(P>0.05)。结论:体现JCI核心理念的社区医疗诊疗流程,不仅能使社区的管理与国际科学接轨,而且按流程运作下的全科诊疗新模式对提升社区的医疗安全性及患者的满意度都具有现实意义。
Objective: To evaluate the implementation results of the community general practitioners' medical service behaviors and the patient's satisfaction in the process of community outpatient diagnosis and treatment according to the community health service process set with the JCI standards. Method: The medical personnel were trained on "patient - centered" core concept of the JCI standards and criterion of the community health service process. The medical behavior of the out- patient doctors and the patient's satisfaction were surveyed and analyzed with homemade questionnaire before and after training. Result: After training, the behaviors for doctors asking the patients medical history, conducting physical examination, telling diseases inform to the patients, and the patient satisfaction were markedly improved comparing them before the training, whose difference had statistical significance (P 〈 0.01). The patient medication adherence and regularity had no obviously changes after treatment before and after the training, whose difference had no statistical significance (P 〉 0.05). Conclusion: The community diagnosis and treatment process which reflects the core concept of JCI has the real significance that not only can the community management integrate with the international science, but also enhance the community medical safety and the patients' satisfaction under the new mode of general medical services in the process.
