摘要
目的通过检测系统性红斑狼疮（SLE）患者血清中同型半胱氨酸（Hcy）的水平，明确其与SLE患者病情活动及狼疮肾炎的关系。方法分别检测40例SLE患者及35例健康人群的血清Hcy水平，并根据病情活动情况、肾脏有无受累分为SLE活动组、非SLE活动组及狼疮肾炎（LN）组、非狼疮肾炎组，分别比较各组Hcy水平及其与病情活动、肾脏受累的关系。结果（1）SLE患者病情活动组、非活动组、对照组血清Hcy水平分别为（16．08±4．97）、（10．86±2．75）和（9．21±2．47）μmol／L，差异具有统计学意义（P〈0．05）；（2）LN组、非LN组、对照组血清Hcy分别为（18．12±4．57）、（11．62±5．17）和（9．21±2．47）μmol／L；（3）抗ds-DNA阳性组、阴性组血清Hcy水平分别为（18．53±4．17）、（12．79±4．78）μmol／L，差异具有统计学意义（P〈0．05）；（4）血清Hcy水平与SLEDAI评分、c反应蛋白（CRP）、血肌酐（Scr）、24小时尿蛋白定量呈正相关，与补体C3、C4、GFR呈负相关。结论Hcy水平与SLE患者病情活动程度相关，并可作为预测其早期肾脏受累的指标。
Objective To evaluate the relationship of serum homocysteine （Hcy） level in pa- tients with system lupus erythematosus （SLE） activities and lupus nephritis. Methods 40 SLE patients and 35 healthy people were detected in serum Hcy levels, and were divided into SLE activity group, no ac- tivity group, lupus nephritis group and no lupus nephritis group according to the illness activities and whether had involved lupus nephritis. Results （ 1 ） The level of serum Hcy in SLE group was significantly higher than in the normal control group（P 〈 0.05 ） ;The serum Hcy level was increased of SLE activity group compared to the no activity group and the difference was statistically significant （ P 〈 0.05 ）. （2） Compared with no LN group and the normal control group, the serum Hcy of LN group was significantly in- creased（P 〈 0.05）. （3）The positive anti-ds-DNA group had higher levels of serum Hcy compared with negative group, the difference was statistically significant （ P 〈 0.05 ）. （ 4 ） The level of serum Hcy and SLEDAI score, CRP（ C-reactive protein）, Serum creatinine（Scr） ,24 h urine protein were positively corre- lated, and C3 , C4, GFR were negatively related to serum Hcy level. Conclusion Serum Hcy levels is re- lated with SLE disease activity and can be used as a predictor of early renal disease in SLE patients.
