摘要
目的探讨经阴道子宫输卵管三维超声造影(three—dimensionalhysterosalpingo—contrastsonography,3DHy—CoSy)在输卵管源性不孕症诊疗中的应用价值。方法选择不孕症妇女104例,将双腔子宫输卵管造影管置入宫腔,经导管推注稀释SonoVue超声造影剂,采用经阴道三维超声造影技术评估输卵管通畅性。并继续实时二维观察双侧的通畅情况,其中2例行腹腔镜检查,30例行子宫输卵管X线造影(HSG)。结果56例双侧通畅;10例双侧阻塞;8例双侧通而不畅;18例一侧通畅,一侧阻塞;12例一侧通畅,一侧通而不畅;本组2例与腹腔镜对照符合。30例与HSG对照,输卵管通畅符合率100%,输卵管通而不畅符合率50%,输卵管阻塞符合率67%。结论经阴道3DHyCoSy能真实展现输卯管的空间走行,并能准确判断输卵管的通畅性,可作为一种新的一线筛查不孕症患者输卵管通畅性的有效手段。
Objective To CoSy) in fallopian tube infertility assess the clinical value of three-dimensiona Methods One hundred and four patients with hysterosalpingo-contrast sonography (3D Hy- nfertility underwent 3D HyCoSy examination. A- coustic contrast agent was injected into the cavity of uterus through the double cavity catheter. Three-dimensional transvaginal ultrasound techniques and continued real-time two-dimensional observation was performed to assess the fallopian tubes. Two cases were examined by laparoscopy and 30 cases were also examined by hysterosalpingography ( HSG ) later. Results The 3D HyCoSy findings showed bilateral patency in 56 cases, bilateral obstruction in 10 cases, bilateral partial obstruction in 8 cas- es; unilateral obstruction in 18 cases and unilateral partial obstruction in 12 cases. The 3D HyCoSy results were consistent with laparoscopic findings in 2 patients. In 30 patients who also received HSG examinations, the consistence rates for Fallopian tube patency, blocked Fallopian tubes and Fallopian tube obstruction were 100%, 50% and 67%, respectively Transvaginal 3D HyCoSy can accurately display the fallopian tube space conditions and fallopian tube patency, wh used as a new first-line examination for screening of infertility patients.
