摘要
目的观察中医证候预测慢性乙型肝炎患者干扰素抗病毒治疗效果的实用价值。方法选择符合入组标准的慢性乙型肝炎患者120例,根据患者证候分为湿热蕴结型、肝郁脾虚型、瘀血瘕积型、肝肾阴虚型4组。所有患者均接受聚乙二醇α-2a干扰素180μg治疗48周,观察患者治疗前及治疗后4、12、36、48周及停药后24周ALT复常率、血清HBsAg水平及HBV-DNA转阴率。结果停药24周时肝郁脾虚组ALT复常率37.9%明显高于肝肾阴虚组的20.7%。治疗第48周、停药24周时,肝郁脾虚组血清HBV-DNA低于检测值率分别为69.0%、69.0%,均高于瘀血瘕积组的39.3%、39.3%,差异有统计学意义(P<0.05)。结论不同中医证候的慢性乙型肝炎患者其干扰素疗效不同,中医证候预测慢性乙型肝炎干扰素疗效具有一定的实用价值,可扩大样本进一步验证以便临床推广。
Objective To observe the practical use on predicting the therapeutic effect of interferon on chronic hepati- tis B by TCM syndromes. Methods According to differentiation of symptoms, 120 chronic hepatitis B patients meet the standards were divided into four types including damp-heat type, liver stagnation and spleen deficiency syndrome, blood stasis type, liver and kidney yin deficiency. All patients were treated with peginterferon alfa-2a Solution for In- jection for 48 weeks. Before and the 4th, 12th, 36th, 48th week after treatment and the 2gth week after completion of medication, the patients were checked serum ALT, HBeAg and HBV-DNA. Results Twenty four weeks of liver depression and spleen deficiency group ALT recovery rate of 37.9% was significantly higher than that of liver and kidney yin deficiency group 20.7%. Treatment for forty-eighth weeks, 24 weeks, liver stagnation and spleen deficiency of serum HBV-DNA values below the detection rate is respectively 69.0%, 69.0%, were higher than those of blood stasis group 39.3%, 39.3%, the difference was statistically significant （P 〈 0.05）. Conclusion The effect of different TCM syn- dromes showes variances, TCM syndromes have a certain practical value on predicting the therapeutic effect of interferon on chronic hepatitis B, further test and verify through larger sample sizes are needed so as to spread in clinical.
