影响高同型半胱氨酸血症型高血压患者血管弹性及内皮功能的相关因素 被引量:23

Correlation factors of arterial stiffness and endothelial function in hypertensive patients with hyperhomocysteinemia

摘要 目的探讨影响高同型半胱氨酸血症(HHcy)型高血压(H型高血压)患者血管弹性、内皮功能的相关因素。方法按血清Hcy水平将105例原发性高血压患者分为H型高血压组(66例)和单纯高血压组(39例),检测两组患者血管弹性、内皮功能,并分析其影响因素。结果 H型高血压组血Hcy、超敏C反应蛋白(hs-CRP)、不对称二甲基精氨酸(ADMA)水平、尿微量白蛋白/肌酐及踝-臂脉搏波反射指数(baPWV)均显著高于单纯高血压组(P<0.05~0.01)。血清Hcy水平与年龄、尿微量白蛋白/肌酐、baPWV、ADMA水平呈正相关(P<0.05~0.001)。baPWV与年龄、吸烟史、糖尿病、hs-CPR、ADMA及Hcy水平呈正相关(P<0.05~0.001)。多因素回归分析显示,baPWV与年龄、糖尿病、Hcy水平及ADMA独立相关(均P<0.05)。结论 HHcy可加重高血压患者血管弹性减退及内皮功能损伤。 Objective To study the correlation factors of arterial stiffness and endothelial function in hypertensive patients with hyperhomocysteinemia (HHcy) ( H-type hypertensive). Methods According to the serum homocystein (Hcy) levels, 105 essential hypertension patients were divided into H-type hypertension group (66 cases) and hypertension group (39 cases ). The arterial stiffness and endothelial function were detected, and the correlation factors of which were analyzed. Results The levels of serum Hcy, high-sensitivity C-reactive protein ( hs- CRP) , asymmetric dimethylarginine (ADMA) , urinary albumin/creatinine and brachial-ankle pulse wave velocity (baPWV) in H-type hypertension group were significantly higher than those in hypertension group (P 〈 0. 05 - 0. 01 ). There were positive correlation between the level of Hcy and age, urinary albumin/creatinine, baPWV, the level of ADMA (P 〈0. 05 -0. 001 ). There were positive correlation between baPWV and age, smoking, diabetes, the levels of hs-CPR, ADMA and Hey (P〈 0. 05 -0. 001 ). Multiple regression analysis showed that the age, diabetes, levels of Hcy and ADMA were independent correlation with baPWV ( all P 〈 0. 05 ). Conclusion HHey can aggravate the vascular elasticity reduced and the vascular endothelial function damage in H-type hypertensive patients.
