乳腺钼靶和彩超对不同大小、不同年龄的乳腺癌的诊断价值比较 被引量:17

摘要 目的探讨乳腺钼靶和超声对不同大小、不同年龄患者乳腺癌的诊断价值。方法对158例乳腺病例经手术证实,与病理结果对照分析,其中67例乳腺癌,91例良性病变,术前比较并分析乳腺钼靶和超声对不同大小、不同年龄诊断乳腺癌的敏感性、特异性。结果①超声诊断乳腺癌随着病灶增大敏感性提高,而钼靶诊断乳腺癌随着病灶增大特异性降低;②钼靶对临床触诊阴性的乳腺原位癌,敏感性明显高于彩超;③钼靶及彩超联合诊断后,对病灶大于1cm的敏感性明显大于钼靶,对不同年龄阶段的诊断乳腺癌敏感性优于钼靶;④彩超对任何年龄阶段,诊断乳腺癌的敏感性无明显差异。结论①钼靶诊断乳腺癌的敏感性与年龄大小因素有关,而超声诊断乳腺癌敏感性与年龄大小无关;②钼靶与超声诊断乳腺癌敏感性及特异性均与可触及肿块大小有关;③乳腺钼靶和彩超相结合对不同大小、不同年龄的乳腺癌可互补不足,从而提高不同大小、不同年龄的诊断乳腺癌的敏感性及特异性。 Objective To explore the value ofmammography and ultrasonography in the diagnosis of breast cancer in different size, different age patients. Methods 158 cases of breast diseases were confirmed by operation and compared with pathological results, of which 67 cases of breast cancer, 91 cases of benign lesions. The sensitivity and specificity of marmnography and ultrasonography for diagnosis of breast cancer with different size and age before the operation were analyzed and compared. Results ①The sensitivity ofultrasonography in the diagnosis of breast cancer increased with enlargement of the lesions, while the specificity of mammography decreased with enlargement of the lesions. ②The sensitivity of molybdenum target for nonpalpable breast cancer was higher than the color Doppler ultrasound. ③The sensitivity of color Doppler ultrasound combined with molybdenum target for focus more than 1 cm and different age phase was significantly higher than molybdenum target. ④The sensitivity of color Doppler ultrasound for the diagnosis of breast cancer at any age had no obvious difference. Conclusions ①The sensitivity of molybdenum target is related to the age factor, and the sensitivity of ultrasound is not. ②The sensitivity and specificity of mammography and ultrasonography are related to palpable mass size. ③Mammography and color Doppler ultrasound combined with different size, different age of breast cancer can be complementary, thereby improving the sensitivity and specificity in the diagnosis of breast cancer with different sizes and different ages.
