摘要
目的观察隔物温和灸联合甲氨蝶呤对类风湿关节炎(rheumatoid arthritis,RA)患者血小板的影响,并探讨其可能机制,为隔物温和灸治疗疾病的机理研究提供一种思路,同时为RA的治疗提供一种有效的方法。方法将40例患者随机分为治疗组和对照组,每组20例。对照组口服甲氨蝶呤,治疗组采用隔物温和灸联合药物治疗,药物治疗同对照组。对患者治疗前后临床症状及血小板计数(PLT)、血小板压积(PCT)、血小板平均体积(MPV)、血小板分布宽度(PDW)进行观察评估。结果共37例患者最终完成试验,治疗组19例,对照组18例。治疗3个月后,治疗组和对照组的临床症状和实验室指标差异有统计学意义(P<0.05)。治疗组19例患者中,7例患者血小板计数异常升高;对照组18患者中,5例患者血小板计数异常升高。治疗3个月后,两组患者血小板参数水平均降低,但治疗组降低程度较对照组显著。结论隔物温和灸联合甲氨喋呤治疗RA疗效明显,可较快地控制疾病的发展。隔物温和灸可能有降低患者异常升高血小板的作用。
Objective To investigate the effect of indirect gentle moxibustion on platelet in rheumatoid arthritis （RA） patients, explore its possible mechanism, offer a thinking way for the study of the mechanisms of indirect gentle moxibustion treatment for diseases and provide an effective method of treating RA. Method Forty patients were randomly allocated to treatment and control groups, 20 cases each. The control group took methotrexate orally and the treatment group received indirect gentle moxibustion plus the same medication as for the control group. The symptoms, platelet count （PLT）, thrombocytocrit （PCT）, mean platelet volume （MPV） and platelet distribution width （PDW） were evaluated in the patients before and after treatment. Result The trial was completed in a total of 37 patients, 19 from the treatment group and 18 from the control group. After three months of treatment, there were statistically significant differences in the clinical symptoms and laboratory indices between the treatment and control groups （P〈0.05）. Of the 19 patients of the treatment group, 7 had an abnormally high platelet count and of the 18 patients of the control group, 5 had an abnormally high platelet count. After three months of treatment, platelet parameter levels decreased in both groups but more in the treatment group than in the control group. Conclusion Indirect gentle moxibustion plus methotrexate has a markedly therapeutic effect on RA and can more quickly hinder the development of disease. Indirect gentle moxibustion may have a reducing effect on abnormally increased platelets in the patients.
