摘要
目的探讨食管癌合并2型糖尿病患者围术期处理原则和方法。方法选取食管癌合并2型糖尿病患者35例(糖尿病组),在围术期规范使用胰岛素,严格控制血糖后进行手术治疗;另选取同期未合并糖尿病的食道癌患者35例为对照组行手术治疗。比较两组术后恢复情况及并发症发生率。结果糖尿病组根治性切除30例,晚期患者已不能手术行胃造瘘术5例,均能平稳渡过手术关,术后并发心律失常3例,乳糜胸1例,切口感染2例,吻合口漏5例,肺部感染4例。对照组根治性切除28例,晚期患者已不能手术行胃造瘘术7例,术后并发心律失常2例,乳糜胸1例,切口感染1例,吻合口漏2例,肺部感染2例。两组患者术后恢复情况及并发症的发生率比较差异均无统计学意义(P>0.05)。结论食管癌合并2型糖尿病患者加强围术期治疗,严格控制血糖,可以达到与对照组一致的围术期治疗效果。
] Objective To explore the principle and methods of perioperative management of esophageal car- cinoma patients with type 2 diabetes mellitus. Methods In diabete group 35 esophageal carcinoma patients with type 2 diabetes mellitns were selected and insulin was regtdarly and strictly used to control patients＇ blood glucose during perioperative period. The control group consisted of 35 esophageal cancer patients without diabetes mellitus receiving operation. Results In diabete group, there were 30 patients who underwent radical resection and 5 patients in ad- vanced stage who underwent gastrotomy ; all the patients passed the perioperative period smoothly. After surgery ,there were 3 patients with arrhythmia, 1 patient with ehylothorax,2 patients with incision infection,5 patients with anastomotie leakage ,4 patients with pulmonary infection ;In control group, there were 28 patients who underwent radical resection and 7 patients in advanced stage who underwent gastrotomy. After surgery, there were 2 patients with arrhythmia, 1 patient with chylothorax, 1 patients with incision infection,2 patients with anastomotic leakage,2 patients with pulmonary infection. In the comparison of postoperative recovery and complication rate, there was no significant differ- ence between two groups （ P 〉 0. 05 ）. Conclusion In the perioperative treatment effect,the diabete group is similar to control group,if perioperative treatment is strengthened and blood glucose is strictly controled.
