摘要
目的观察锡类散对肥皂液灌肠造成的兔肠黏膜损伤的保护作用。方法将40只家兔随机分成两组各20只,分别用肥皂液(A组)、肥皂液+锡类散(B组)灌肠。观测各组灌肠前后血钾水平变化以及粪便涂片肠黏膜细胞脱落情况;在腹腔麻醉下取直肠、结肠远心回、结肠近心回肠壁制作病理切片,电镜下观察肠黏膜细胞亚显微结构变化;光镜观察肠黏膜充血、水肿情况。结果A组灌肠后血钾水平显著低于B组(P<0.01)。两组直肠部位肠黏膜损伤最重,结肠远心回其次,结肠近心回最轻;A组直肠部位以及结肠远心回肠黏膜损伤程度较B组严重(均P<0.01)。A组可见肠黏膜细胞损伤较严重,线粒体、内质网肿胀,细胞连接增宽,见大量凋亡、坏死细胞。B组肠黏膜细胞亚显微结构基本无改变或轻度损伤,细胞质内细胞器丰富,结构未见异常,细胞连接紧密,固有层腺体结构正常。A组肠黏膜细胞脱落情况较B组严重。结论锡类散对碱性灌肠液造成的兔肠黏膜损伤具有一定保护作用。
Objective To explore the preventive effect of traditional Xilei powder on intestinal mucosal injury caused by soapsud ene- ma in rabbits. Methods Forty rabbits were randomized into 2 groups of 20 each. Group A received soapsud enema,while group B were given enema solution containing soapsud and Xilei powder. Serum potassium was detected before and after enema,and exfolia- tive cells were observed in stool smear. Then, samples were taken from the lumen wall of rectum,and distal and proximal colon for pathological evaluation when the rabbits were under intraperitoneal anesthesia. Submicroscopic structure of intestinal cells was ob- served through electron microscope and congestion and edema of intestinal mucosa were observed through optical microscope. Re- salts The concentration of serum potassium in group A after enema was significantly greater than that in group B (P〈0.01). The intestinal mucosa harvested from the rectum sustained the most severe injury in the 2 groups, followed by mucosa collected from distal and proximal colon. The intestinal mucosa harvested from the rectum and distal colon in group A were more severely injured than in group B (P〈0.01 for both). The mucosal cell injuries in group A were critical,the mitochondrion and endocytoplasmic re- ticulum appeared edematous; cell junction was broadened, and lots of exfoliated and necrotic cells were found. On the contrary, the mucosal cells in group B looked morphologically normal with distinct structural layers as well as tight cell junction. The lamina pro- pria was normal, and there were less eolorectal exfoliative cells from stool smear. Conclusion Xilei powder has some protective effect on injuries of intestinal mucosa caused by soapsud enema.
