类风湿关节炎患者DAS28积分与疾病活动指标的相关性分析被引量：60

摘要目的分析类风湿关节炎(RA)患者临床常用疾病活动指标与28个关节疾病活动度评分(DAS28积分)的相关性,探讨反映DAS28积分的敏感指标。方法以贵阳中医学院第二附属医院就诊的258例符合统一诊断标准的门诊和住院RA患者为研究对象,观察RA患者的临床常用疾病活动指标,包括DAS28积分、关节功能分级、有无关节腔积液、贫血程度、C-反应蛋白(CRP)、血沉(ESR)和血小板(PLT),分析其相关性并进行多元逐步回归分析。结果关节功能分级、贫血程度、CRP、ESR、PLT与DAS28积分做相关分析,相关系数分别为0.297、0.357、0.268、0.592、0.442(P<0.01),其中ESR与DAS28积分的相关性最高。多元逐步回归分析发现,关节功能分级、ESR和PLT与DAS28积分呈正相关,标准化回归系数分别为0.474、0.128、0.119,(P<0.05);有无关节腔积液与DAS28积分呈负相关,标准化回归系数为-0.188(P<0.01);其中ESR是DAS28积分的最重要预测因素。结论临床可通过监测ESR了解患者的疾病活动度,同时也不能忽视患者关节的症状、体征。
