摘要
目的回顾性分析医院感染产超广谱β-内酰胺酶(ESBLs)细菌的危险因素,并提出相应的预防与控制对策。方法收集2005年1月-2011年10月患者痰标本,采用K-B琼脂扩散法和ESBLs确证试验进行ESBLs的检测,医院感染产ESBLs菌危险因素的分析采用多因素logistic回归分析法。结果共检出肠杆菌科细菌946株,其中产ESBLs细菌238株,检出率25.2%;多因素logistic回归分析结果,年龄(OR=2.66,P=0.021)、基础疾病(OR=2.76,P=0.023)、机械通气(OR=6.72,P=0.013)、三代头孢菌素应用(OR=3.88,P=0.024)、住院时间(OR=2.11,P=0.031)全部进入回归方程。结论年龄、基础疾病、机械通气、三代头孢菌素应用、住院时间为医院感染产ESBLs菌的独立危险因素,积极治疗基础疾病、加强患者的营养支持、制定合理的个性化的预防方案对于控制和减少肺部感染产ESBLs菌具有积极的意义。
OBJECTIVE To investigate the risk factors for the pulmonary infections caused by extended spectrum β-lactamase-producing bacteria so as to propose the corresponding prevention and control measures.METHODS The sputum specimens of the patients were collected from Jan 2005 to Oct 2011,the ESBLs were tested by Kirby-Bauer(K-B) method and the ESBLs confirmatory test.The multiple logistic regression analysis was performed for the risk factors for ESBLs-producing bacteria infections.RESULTS Of totally 946 strains of Enterobacteriaceae isolated,there were 238 strains of EBBLs-producing strains with the detection rate of 25.2%.The multivariate logistic regression analysis revealed that the age(OR=2.66,P=0.021),underlying diseases(OR=2.76,P=0.023),and the use of third generation of cephalosporins(OR=3.88,P=0.024),and the hospital stay(OR=2.11,P=0.031) all involved the regression equation.CONCLUSION The age,underlying diseases,mechanical ventilation,use of the third generation of cephalosporins,and the hospital stay are the independent risk factors for the nosocomial infections caused by the ESBLs-producing bacteria.It is of great significance in the control and reduction of the ESBLs-producing strains to actively treat the underlying diseases,strengthen the nutrition support,and work out a reasonable and personalized prevention program.
