不孕妇女的年龄和基础FSH水平对夫精人工授精结局的影响 被引量:7

The impacts of age and basal follicle-stimulating hormone levels on outcomes of infertile women undogoing artifical insemination by husband′s sperm

摘要 目的探讨年龄和血基础促卵泡素(FSH)水平对不孕妇女夫精人工授精结局的影响。方法回顾性分析我院生殖健康科2006年1月~2011年12月接受夫精人工授精治疗的不孕患者的年龄和基础FSH水平与临床妊娠率和流产率的关系。结果共计257例,535周期。年龄<35岁组临床周期妊娠率为15.1%,≥35岁组为13.2%(P>0.05);年龄<35岁组流产率为19.0%,低于≥35岁组的50%(P<0.05)。在<35岁组,促排卵周期妊娠率(25.0%)高于自然周期(11.1%)(P<0.05),但在≥35岁组,其差异无统计学意义。基础FSH<10IU/L组与≥10IU/L组妊娠率无显著性差异,但流产率降低(22.4%vs.54.5%,P<0.05)。FSH<10IU/L组促排卵周期妊娠率为23.1%,高于自然周期组的12.3%(P<0.05),而FSH≥10IU/L组促排卵周期妊娠率与自然周期妊娠率无显著性差异。结论高龄和基础FSH升高的不孕妇女夫精人工授精妊娠率下降,而流产率上升,促排卵不能增加高龄和高FSH患者的人工授精妊娠率。 Objective: To explore the impacts of age and basal follicle-stimulating hormone(FSH) levels on outcomes of infertile women undergoing artifical insemination by husband′s sperm(AIH).Methods: The correlations between age or basal FSH levels and clinical pregnancy rates(PR) and miscarriage rates(MR) were analyzed retrospectively in infertile women undergoing AIH in our department between 2006 and 2011.Results: Total of 535 cycles.There was no significant difference in PR between35 age-group and ≥35 age-group(15.1% vs.13.2%,P0.05),but the MR was significantly lower in35 age-group than in ≥35 age-group(19.0%vs.50.0%,P0.05).Moreover,the PR was significantly higher in ovarian stimulating cycles(25.0%) than natural cycles in35 age-group(11.1%)(P0.05).The elevated FSH group was found with a significant higher MR than the normal FSH group(54.5% vs.22.4%,P0.05),in which the PR was significantly higher in ovarian stimulating cycles(23.1%) than in natural cycles(12.3%)(P0.05).Conclusion: A elevated FSH or age in infertile women undergoing AIH therapy will result in a low PR and high MR,which cannot be improved by ovarian stimulation.
