摘要
目的了解肠球菌属的检出率和临床分布特点及耐药状况。方法收集医院2011年1月~2012年6月常规培养检出的286株肠球菌属,分析其在各种临床标本中的分布特征、耐药状况、高浓度氨基糖苷类耐药状况。结果检出的286株肠球菌属中,屎肠球菌166株(58.0%)、粪肠球菌96株(33.6%)、其他肠球菌共24株(8.4%),临床标本主要来自尿液和痰液,尿液149株(52.1%)、痰液59株(20.6%);屎肠球菌对青霉素、氨苄西林、红霉素、利福平、环丙沙星、呋喃妥因、高浓度庆大霉素、高浓度链霉素耐药率均〉85.0%,粪肠球菌对红霉素、四环素、利福平的耐药率均〉75.0%,万古霉素和利奈唑烷的耐药率为0.0%;筛选出高浓度氨基糖苷类药物耐药株210株(73.4%)。结论肠球菌属感染以屎肠球菌和粪肠球菌为主,可引起泌尿和呼吸系统等多部位感染,屎肠球菌耐药性高于粪肠球菌;肠球菌多药耐药和高耐现象相当严重。
Objective To study the detection rate, clinical distribution characteristics and drug resistance of enterococcus. Methods Two hundred and eighty six strains of enteroeoccus were collected in the conventional culture of our hospital from January 2011 to June 2012. Clinical distribution characteristics, drug resistance, resistance to a high concentration of amino glycoside were analyzed. Results Among the 286 strains, enterococcus faecium accounted 166 (58.0%), enterococcus faeealis accounted 96 (33.6%), and other enterocoeeus accounted 24 (8.4%). These strains mainly came from urine and sputum, with urine and 149 strains (52.1%), and sputum 59 strains (20.6%). Enteroeoecus faecium had a resistance rate higher than 85.0% to penicillin, ampicillin, erythromyein, rifampin, ciprofloxacin, nitrofurantoin, high concentration of cidomycin and high concentrations of streptomycin. Enterococeus faecalis had a resistance rate higher than 75.0% to erythrornycin, tetracycline and rifampin. 0.0% resistance were showed to vancomycin and linezolid. 210 (73.4%) strains were resistant strains to high concentration of aminoglyeoside. Conclusion Enterococcus infections are mainly enteroeoeeus faecium and Enterococeus Faecalis and can induce multi-site infection especially in urinary and respiratory system. Enteroeoeeus faeeium has a higher resistant rate to drugs than enterocoeeus faecalis. Multi-drug resistance and high resistance phenomenon in enterococeus is very serious.
