摘要
目的探讨双路泪道引流管泪道置管结合冲洗液在泪道断离中的临床应用疗效。方法回顾性分析2008年10月～2012年4月在山东省垦利县人民医院五官科住院治疗的外伤性泪小管离断行一期高倍显微镜下泪小管离断吻合术患者46例46眼,在泪道断离患者行泪道断离吻合术中分双路从上下泪道中置入泪道引流管并在鼻腔前端打结,常规行断端吻合术,术后7d开始用自制冲洗液(氧氟沙星滴眼液5mL+地塞米松1mL+糜蛋白酶4000U)冲洗泪道,1周1次,至术后3个月拔除泪道引流管后继续冲洗2～4周,观察手术疗效及术后并发症发生情况。结果全部患者均手术一次成功,随访1个月至半年。46例中治愈44例(无溢泪,冲洗泪道通畅),治愈率为95.7%,无明显并发症发生。结论采用双路从上下泪道中置入泪道引流管常规行断端吻合术并结合自行配制的泪道冲洗液冲洗泪道治疗泪道断离患者疗效确切,值得临床推广应用。
Objective To explore the effect of two way lacrimal duct drainage tube and homemade rinses in lacrimal duct trauma. Methods Forty-six patients of lacrimal duct trauma in our hospital were selected and treated with lacrimal duct broken end anastomosis from october of 2008 to April of 2012, the two way lacrimal duct drainage tube and homemade rinses （ofloxacin eye drops 5 mL＋hexadecadrol 1 mL＋chymotrypsin 4 000 U, once/week） were used, to 3 moths after remove lacrimal duct drainage tube continue to rinse after 2 to 4 weeks and the effect and the complications were observed and analyzed. Results The operation of all the patients were successful and were followed up from one month to six months, 46 cases of cure 44 cases （no tears overflow, flushing lacrimal duct flow）, and cure rate was 95.7% （44/46）, and the complications were no happened. Conclusion Two way lacrimal duct drainage tube and Homemade rinses treated in lacrimal duct trauma has a good effect and it is worthy of application .
